This article covers the particular challenge of protecting the heart compromised from infarction. The Buckberg and Emory reperfusion protocols are outlined and their merits discussed.

Hammermeister KE, Burchfiel C, Johnson R, Grover FL. Identification of patients at greatest risk for developing major complications at cardiac surgery. Circulation 1990 82(5 Suppl):IV380-9.

Analysis of over 10,000 patients shows that previous cardiac surgery is a strong predictor for the development of postop complications, and patients who develop complications have a 8-10 fold increase in operative mortality.

Although such events are uncommon, this series demonstrates a correlation between prolonged CPB time and development of abdominal complications. As expected, mortality is higher than in those patients without such problems.

This retrospective review compares retrograde cardioplegia to combined antegrade/retrograde cardioplegia. The techniques were essentially equivalent for myocardial protection.
