HEMODYNAMIC EVALUATION OF THE JARVIK 2000 HEART DURING ACUTE HEART FAILURE

Purpose and background: The Jarvik 2000 Heart is designed for long term left ventricular assist. This study aimed to evaluate the level of hemodynamic support provided by this pump in an animal model with acute heart failure.

Methods: Five calves were used. Heart failure was induced by progressively ligation of the LAD combined with a continues infusion of Esmolol. The pump was turned on and hemodynamic measurement was obtained at different pump settings ranging from 8000 to 12000 RPM.

Results: CI, LAP and systemic arterial blood pressure improved significantly with a pump speed over 9000 RPM. Pulse pressure narrowed by 64 %, PAP remained unchanged. Other hemodynamic parameters as heart rate, SVR, PVR and CVP remained unchanged.

Conclusion: The Jarvik 2000 Heart can provide a level of cardiac output support that normalises the CI and LAP while maintaining some arterial pressure pulsatility during acute heart failure.

Systemic perfusion is increased while the left heart is unloaded.
