The aim of this paper is to compare the immediate follow-up of patients (pts) operated upon with and without cardiopulmonary bypass (CPB) .

We divided 239 pts submitted to CABG in 3 groups (G): G I - OPCAB with aortic approach (63 pts);  G II - OPCAB with no aortic approach (64 pts); G III – CABG with CPB (95 pts). The number of bypass/pt  was 2.17, 1.16 and 3.27 respectively and reoperations, emergency surgery and pts over 80 years were excluded.

Total morbidity was 7.9%, 12.5% and 13.7%, respectively (p=0.225). Regarding neurological complications, there were 2 (2.1%) strokes in G III and none in the other groups (p=0.017). The mortality rate was : 3.17% in the G I; zero in the G II  and 3.16% in the G III (p=0.472).

In conclusion, in these pts the OPCAB groups presented less neurologic complications when compared with CPB group and the total morbidity and mortality rates were not reduced when compared the 3 groups.
