OFF-PUMP CORONARY ARTERY BYPASS GRAFTING SIMULTANEOUSLY
COMBINED WITH VASCULAR SURGERY

Objective: We describe our experience with off-pump coronary
artery bypass grafting (OPCAB) simultaneously combined with vascular
surgery.
Patients & Methods: Between March 1997 and August
2000, OPCAB combined with vascular surgery were performed in 18
patients (14 males, 4 females; mean age, 71 years). Combined vascular
procedures included repair of infrarenal abdominal aortic aneurysm
in 14 patients, repair of thoraco-abdominal aortic aneurysm (Crawford
IV) in 1 patient, axillo-bilateral femoral bypass in 1 patient,
femoro-popliteal bypass in 2 patients. In 10 patients LITA-LAD
was performed through a left anterior small thoracotomy. In 8
patients OPCAB using Octopus was performed through median sternotomy.
Results: Twenty-seven anastomoses were performed (1-4
graft / pt). No patients had conversion to CPB or perioperative
myocardial infarction. One (5.5%) patient died postoperatively
due to interstitial pneumonia. In the other patients, the postoperative
courses were uneventful. There was neither angina recurrence nor
other morbidity during the follow up periods (1-41 months).
Conclusion: OPCAB simultaneously combined with vascular
surgery was a safe and useful procedure.
