Once you have completed the form in its entirety, you will find a submit tab at the bottom of the form. By clicking the submit tab your clinical question will be forwarded to the STSDB-FAQ@STS.org mailbox.

• Questions will be reviewed and the answer will be e-mailed back to you within 30 days
• Questions will be answered in the order that they are received
• Only one question per STS Clinical Question Request Form
• Please do not re-submit questions

Questions pertaining to harvest timelines, verifying data receipts, resolving data transmission problems, clarifying data quality report concerns, monitoring data specification questions and interpreting final report matters should be sent to your Clinical Data Specialist at DCRI.
