Invitation to Participate in CALGB Trial
Dr. Nasser K. Altorki, The New York Hospital and Cornell Medical Center, New York, NY USA, writes: CALGB has recently activated a randomized trial comparing lobectomy with limited resection for small (2 cm or less) peripheral NSCLC that is node-negative. It calls for randomizing nearly 900 patients. It was endorsed by all the co-operative groups, including AGOSOG and NCI-Canada. Unfortunately, accrual has been slow, and we would like to raise awareness of the trial since it is likely, when completed, to be an important contribution."
To those interested in participating, he writes: "A large randomized trial of lobectomy versus sublobar resection for early stage NSCLC has been activated throughout the US and Canada. Read the Trial Proposal, and see how you can participate."

Thoracic
Dr. Douglas E. Wood, Seattle, Washington USA has designed a survey to investigate this relationship amongst the members of the cardiac and thoracic community, and is obtaining data from cardiothoracic surgeons around the world.
We invite you to participate in a survey that will only take a few minutes of your time. Your participation is important as this will help us to understand the educational background and the professional activity of general thoracic surgeons and their inter-relationship with cardiac surgery and general surgery around the world.
Training and certification for general thoracic surgeons varies enormously between countries. For some, thoracic surgery is a subspecialty of general surgery, in others general thoracic surgeons are trained in parallel but separate from cardiac surgeons, while in others cardiac and thoracic surgery training are united and lead to common certification. This survey’s goal is to outline the types of training and certification for general thoracic surgeons, and specifically looking at the relationship, or lack thereof, between thoracic surgery training and cardiac surgery training around the world. This should only require 5-10 minutes of your time. Responses will remain confidential, and results will be published in the thoracic surgery literature and CTSNet in late 2008.
The questionnaire is available online. For your convenience a copy of the URL is also given in case the hyperlink in this message is not functioning:
www.surveymonkey.com/s.aspx?sm=GzWwwDWSi_2b2h2e2MimQ2UQ_3d_3d.
Thank you for taking the time to answer these few questions to help us understand our specialty better.

International Board of Blood Management
The American Society of ExtraCorporeal Technology (AmSECT) is pleased to announce the establishment of a certifying agency for the recognition of individuals performing autotransfusion as part of a comprehensive blood management program.
The International Board of Blood Management (IBBM) has the mission of promoting education and sound scientific principles to advance the safe and competent practice of perioperative blood management. The IBBM was designed to provide a process to formally recognize individuals who administer blood management services with primary emphasis on autotransfusion. The recognition will occur following the successful completion of requirements listed on the IBBM website (Intbbm.org). The requirements will include completion of an application for certification and the successful completion of an examination on blood management subject matter including autotransfusion techniques. Recertification will be conducted on a yearly basis and include a clinical activity requirement. Successful completion of all requirements for certification will result in certification by the IBBM of the successful applicant as a Perioperative Blood Management Technologist (PBMT). The PBMT is an individual who is currently providing the service of autotransfusion as a component of a blood conservation program. The individual will be knowledgeable in the techniques of autotransfusion as well as blood management subject matter which will include hematology, physiology, mechanical sciences and regulatory mandates by national and federal organizations.

ATS April 2008: Pizarro C et al. Stage II Reconstruction After Hybrid Palliation for High-Risk Patients With a Single Ventricle. Ann Thorac Surg 2008;85:1382-1388.
The hybrid approach to palliate high-risk neonates with a single ventricle and systemic outflow obstruction continues to gain interest. Despite early success, few data exist regarding the physiologic adequacy of this palliation and the outcome of the stage II reconstruction. The authors reviewed their experience with stage II reconstruction after hybrid palliation in 14 newborns with hypoplastic left heart syndrome and variants. Among 14 patients undergoing hybrid palliation, the median Qp/Qs was 0.76, and pulmonary artery pressure was 14 mm Hg. Stage II reconstruction was performed in 8 patients with a median age of 4 months. Median cardiopulmonary bypass time was 124 minutes (range, 95 to 188 minutes). Median time to extubation was 20 hours (range, 9 to 120 hours). Median oxygen saturation at hospital discharge was 79% (range, 78% to 82%). Two perioperative deaths occurred. To date, all hospital survivors are well. Four patients have completed a Fontan.
The authors have concluded that Stage II reconstruction after hybrid palliation for high-risk neonates carries important morbidity and mortality. A considerable number of reinterventions to optimize the palliated physiology are necessary, but this approach can provide appropriate preparation for single-ventricle management while avoiding cardiopulmonary bypass in the neonate. Additional experience and critical risk assessment of the entire strategy are necessary to define its advantages.
The hybrid approach remains a controversy, but is currently the recommended strategy in approximately 15-20% of centers in North America dealing with HLHS. Two important considerations are case selection and local results with the standard Norwood sequence.
