5th EACTS/ESTS JOINT MEETING

The 5th Joint Meeting of the EUROPEAN ASSOCIATION FOR CARDIO-THORACIC SURGERY (EACTS) and the EUROPEAN SOCIETY OF THORACIC SURGEONS (ESTS) will be held September 9-13, 2006, in Stockholm, Sweden. The EACTS-ESTS Techno-College is September 9. Deadlines: Late registration 18 August, 2006. After this date, on-site fees will be charged. On-site registration is possible.

The American Association for Thoracic Surgery (AATS) is now accepting abstracts for presentation at the 87th Annual Meeting to be held May 5-9, 2007 at the Washington Convention Center in Washington, DC.  The online abstract submitter site contains complete submission instructions and guidelines.  The SUBMISSION DEADLINE IS October 10, 2006.

An Introduction to The American Heart Association's Council on Cardiovascular Surgery and Anesthesia

The American Heart Association (AHA) is the largest volunteer and charitable organization whose mission is to reduce death and disability from cardiovascular diseases and stroke. In 2000, the AHA established a strategic goal of reducing death, disability and risk by 25% by the year 2010. Most people are familiar with the local fund raising efforts of the AHA in terms of heart walks or mini-marathons, school activities such as "Jump Rope for Heart", heart galas and auctions, mailing campaigns, and the more recent "Go Red For Women" programs. The average donation received by the AHA is less than $25. However, the success of the combined fund-raising programs permits support of four major areas from a scientific and health care standpoint: 

The AHA's Council on Cardiovascular Surgery and Anesthesia is one of 13 Scientific Councils. Our mission is to improve the application of established surgical procedures and anesthetic techniques; increase awareness of new issues in the field of cardiovascular surgery and anesthesia; and disseminate new knowledge to increase the number of patients who may benefit from surgical treatment in safer and more effective ways.

Our Council is committed to supporting young investigators. We provide funding for the AHA's Student Scholarships in Cardiovascular Disease and Stroke. We sponsor the Vivien Thomas Young Investigator Awards competition which provides travel stipends and financial awards to those finalists who are chosen to present their work at the annual Scientific Sessions. Earlier this year, we provided a rationale to the NHLBI regarding the importance of the relative time for research versus clinical commitments for our specialties for whom clinical and technical skills are paramount. We were joined in this effort by the Society of Thoracic Surgeons, the American Association for Thoracic Surgery, and the Thoracic Surgery Foundation for Research and Education. We recently received notice that the NHLBI has agreed to reduce the research time percentage from 75% to ?50-75%? for clinically-oriented specialists to apply for K08 and K23 training grants.

Our Council sponsors the Surgical Supplement to Circulation which contains peer-reviewed papers presented at Council-sponsored sessions at the annual Scientific Sessions. Abstract presenters at Council-sponsored sessions may submit papers for review. 

Over 400,000 patients have cardiac and vascular surgery each year. The daily interactions of cardiac and vascular surgeons and cardiovascular anesthesiologists are a natural starting point for our Council to develop new programs and enhance existing ones.

ISMICS Celebrates 10-Year Breakthrough

The International Society for Minimally Invasive Cardiothoracic Surgery (ISMICS) will celebrate its 10th anniversary during its Annual Scientific Meeting scheduled 6-9 June 2007 at the Aurelia Convention Centre & Expo in Italy.  Special guest speakers will address the rapid development and innovation in techniques and technology over the past 10 years and provide insight into the direction technology is taking in the future. The scientific meeting will feature original research presentations, panel discussions, post graduation education and much more.

CALLING 500 TRANSPLANT PHYSICIANS AND SURGEONS!

The most successful donation service areas (DSAs) in the country are transplanting more than 3.5 organs per donor. If your DSA isn't meeting this target you can get involved now.

On October 18-19, 2006 the Organ Transplantation Breakthrough Collaborative (OTBC) National Learning Congress will convene in New Orleans, Louisiana. The OTBC was created to maximize the number of organs transplanted from each donor and to raise the national rate from 3.06 to 3.75. Achieving this goal requires the active involvement of transplant physicians and surgeons throughout the country. The Collaborative is challenging 500 transplant physicians and surgeons to attend a special symposium on October 19th developed specifically to address their role in procuring and utilizing more organs. This one-day meeting will partner transplant physicians with local and national donor hospital intensivists, OPO directors, and coordinators to maximize utilization of hearts and lungs from heart beating and non-heartbeating donors.

The Breakthrough Collaboratives have generated an unparalleled track record of success in increasing the number of organ donors, including back-to-back months with donors in excess of 700 per month. With your active involvement, these same successful results can be generated for the number of organ transplants performed.

AUGUST 2006 ARTICLE FOR DISCUSSION

Polvani G et al: Reduction Ascending Aortoplasty: Midterm Follow-Up and Predictors of Redilation. Ann Thorac Surg. 2006;82:586-91.

In the August 2006 Annals, Polvani et al report their experience with reduction aortoplasty.  They propose the procedure as an alternative to the replacement of the ascending aorta in case of ascending aorta aneurysm without aortic root involvement. The study was designed to evaluate the midterm result and to determine predictors of redilation in 68 patients.  All patients underwent associated surgical procedures,  and 88.2% underwent associated aortic valve replacement. Median follow-up time was 2.5 years. The actuarial freedom from cardiac-related death at 3 and 6 years was 100% and 95.7% +/- 4.3%, respectively. The actuarial freedom from redilation at 3 and 6 years was 97.7% +/- 2.3% and 79.8% +/- 8.4%, respectively. The actuarial freedom from reparation at 3 and 6 years was 100% and 86.3% +/- 7.5%, respectively. Only preoperative diameter was a significant predictor of redilation using multivariate stepwise logistic regression analysis. The authors concluded that unsupported reduction aortoplasty is a safe and effective technique with low mortality, low morbidity, and rare late complications, as applied in cases with ascending aortic diameter less than 55 mm.

To some extent this experience challenges traditional surgical thinking regarding root dilation.  The results are excellent although the follow-up time may not be long enough to generate wide spread acceptance, especially in the face of similar complication-mortality rates for root replacement in many published series.

JTCVS Journal Announcements August 2006

In Statistics for the Rest of Us, this issue of JTCVS spotlights the role of CONSORT (Consolidated Standards of Reporting Trials) for reporting randomized controlled trials (RCTs). Tiruvoipati and colleagues take cardiothoracic journals to task for publishing RCTs that do not comply with CONSORT and for not officially subscribing to it. The diverse reviewers of Tiruvoipati and colleague?s paper have written brief invited commentaries dealing either with issues raised by the paper or with RCTs in general. The result is a stimulating, serious discussion of important issues that RCTs raise in surgery.

Also in this issue, Melby et al present a preclinical study of microwave radiation used to ablate the atrium of the beating heart. In an accompanying editorial, Dr. Saltman discusses 2 overarching questions raised by this type of research: (1) Can ablative energy sources create truly transmural lesions when used from the epicardium on the beating, filled heart and (2) are transmural lesions necessary to cure atrial fibrillation?

The article by Shann et al is an evidence-based review for conducting safe, patient-centered, effective cardiopulmonary bypass (CPB) practice. This manuscript, the first of several underway regarding the development of evidence-based findings for CPB, focuses on neurological protection, euglycemia, hemodilution, and the inflammatory response. It is accompanied by a CME activity.

CARDIOTHORACIC SURGERY SPECIALTY UPDATE COURSE

The Royal College of Surgeons of Edinburgh is conducting a 2-day current state of the art review course in cardiothoracic surgery in Singapore from 17th to 18th October 2006. The 3rd WORKSHOP ON INTEGRATED CORONARY REVASCULARIZATION will be held 6-8 December, 2006 in Innsbruck, Austria.

Our earlier workshops showed that combined coronary intervention carried out by interventional cardiologists and cardiac surgeons in direct cooperation is a realistic option. It was demonstrated that a team approach that also includes cardiac anesthesiologists, cardiac imaging specialists, perfusionists and cardiac nursing specialists is critical for offering the best therapeutic options to patients with coronary artery disease.

We kindly invite all colleagues interested in integrated approaches to treatment of coronary artery disease to our 3rd workshop and strongly encourage attendance of coronary revascularization teams.

Formation and maintenance of such teams will be the main topic of the 2006 meeting. A distinguished national and international faculty will lead attendees through new ways of cooperation between specialties working on invasive treatment of coronary artery disease.
