New AATS Award Program for Medical Students

During its recent meeting, the Council of The American Association for Thoracic Surgery (AATS) approved a new scholarship program for first and second year medical students in North America to introduce these students to the field of cardiothoracic surgery and to broaden their educational experience by providing them with an opportunity to work in a local cardiothoracic surgery program.  Successful candidates will receive a grant of $5,000 to underwrite their living expenses for an eight-week internship in a local cardiothoracic surgery program.

Eligible candidates must complete an online application by JANUARY 31st, 2007, and include a one-page outline of what they hope to accomplish during their eight-week internship, as well as a letter of support from the prospective host sponsor who must be a member of the AATS. 

We ask that you circulate this new program information to all first and second year medical students at your institution and/or in your local area.

Additional award criteria, including an application can be accessed on the AATS website.  THE APPLICATION DEADLINE IS JANUARY 31, 2007.

Poland: The Great Orchestra of Christmas Charity Foundation

Over the last 14 years the GOCC Foundation has motivated hundreds of thousands volunteers to gather nearly $70,000,000 USD for creating complex systems of support for children with special health care needs. This money has equipped 650 hospital wards with 16,000 medical devices. That means that statistically, every single day the GOCC donates 3 devices.

The Foundation created, launched and finances 4 national programs, which became a crucial part of public healthcare: The Program of Universal Neonatal Hearing Screenings, Implementing Insulin Pumps in Therapy of Children with Diabetes, Non-Invasive Breathing Support in Neonates, and the Program of Preventing and Treatment of Retinopathy of Prematurity.

Thanks to their support, congenital heart surgery in Poland is one of the best in the world. Out of 3,000 newborns with congenital heart defects born yearly, 2,700 go through life-saving procedures. The ratio of surgeries for CHD per million population is now the highest in Europe (F. Unger, Salzburg). Fourteen years ago only 10% of children with CHD were operated upon.

In addition, GOCC actively promotes public access to AEDs, and teaches CPR and first aid classes to the kids and youth of Poland.

A driving force behind this endeavor is Dr. Bohdan Maruszewski, Warsaw, Poland. He is Director and Chief Surgeon, Dept. for Pediatric Cardiothoracic Surgery, Children's Memorial Health Institute, Warsaw, Poland, and Database Director for the EACTS Congenital Database.

AATS Has Added a Valuable New Resource

The American Association for Thoracic Surgery (AATS) has made available on its website the Annual Meeting Program books in their entirety from the eleventh meeting (1928) to 2006. Also available are complete lists of past officers, basic science lecturers, honored guest lecturers, and AATS award recipients. THE THORACIC SURGERY FOUNDATION FOR RESEARCH AND EDUCATION

TSFRE is pleased to announce the opening of the 2007 Award funding cycle!
TSFRE is now accepting on line applications for the following awards:

Research Fellowship Awards
Support of up to $35,000 per year for up to two years for surgical residents who have not yet completed cardiothoracic surgical training.

Research Grants
Operational support of original research efforts by cardiothoracic surgeons who have completed their formal training and who are seeking initial support and recognition for their research program. Awards of up to $30,000 a year for up to two years are made each year to support the work of an early-career cardiothoracic surgeon (within five years of first faculty appointment).

Career Development Awards
Salary support of up to $50,000 a year for up to two years for applicants who have completed their residency training and who wish to pursue investigative careers in cardiothoracic surgery.

Nina Starr Braunwald Career Development Awards
Provides a biennial award of $100,000 for two years to support the research career development of a woman cardiac surgeon who holds a full-time faculty appointment and who is within ten years of completion of thoracic surgery residency.

Application Timeline:

Oppido et al. Moderately Hypothermic Cardiopulmonary Bypass and Low-Flow Antegrade Selective Cerebral Perfusion for Neonatal Aortic Arch Surgery.

Although deep hypothermic circulatory arrest has been extensively used in neonates for aortic arch surgery, the brain and other organs might be adversely affected by prolonged ischemia and deep hypothermia. This of course has been controversial and difficult to prove, despite the attractiveness of the concept. Between December 1997 and January 2005, 70 consecutive neonates underwent the Norwood  procedure for hypoplastic left heart syndrome (group A, n = 30), or aortic arch repair for interruption or coarctation with arch hypoplasia (group B, n = 40), with antegrade selective cerebral perfusion (ASCP). Mean weights were 3.0 ± 0.2 kg and 2.8 ± 0.07 kg, and mean ages were 10 ± 3.5 days and 14 ± 10.6 days in groups A and B, respectively. Only 2 patients were older than 30 days. Core body temperature was lowered to 25°C, and mean pump flow during ASCP was initiated at 10 to 20 mL/(kg * min) and adjusted to guarantee a radial/temporal artery pressure of 30 to 40 mm Hg and venous oxygen saturation of more than 70%. Hematocrit was maintained at 30%.

Early mortality was 17% (group A, 23%; group B, 12.5%; p = 0.19). At 36 months, Kaplan-Meier overall survival was 64% ± 9.2% in group A and 85% ± 5.7% in group B. One patient had postoperative seizures. Age, weight, sex, prematurity, group A, and ASCP duration did not influence early mortality. The authors conclude that antegrade selective cerebral perfusion is a safe and effective procedure and might improve outcome of neonatal aortic arch surgery, minimizing neurologic impact without the need for deep hypothermia.

As written, this is a very nice descriptive paper with good results and a thoughtful analysis.  However, as in most other papers on the subject, we lack a prospective analysis that might strengthen the argument. Other questions could be raised as well, for example: best temperature to use, how to cannulate, need for multiple arterial cannulas, best flow, how to assess the postoperative neurological outcome (early and late), etc.

AATS Grantsmanship Workshop 2007 will be held March 2, 2007 from 8:00 a.m. - 5:00 p.m. at the Hyatt Regency Hotel, Bethesda, Maryland, USA.

This workshop, organized by Dr. Pedro J. del Nido of Children's Hospital Boston and Chair of the AATS Scientific Affairs and Government Relations Committee, is designed to help professionals who are applying for grants, including current and previous grant recipients, to better understand the complex nature of preparing and submitting grant proposals.
