What's New

Dr. Andrew Wechsler retires as Editor of JTCVS

After serving 7 years as Editor-in-Chief of the Journal of Thoracic and Cardiovascular Surgery, Dr. Wechsler steps down this month. The December issue contains a tribute to Dr. Wechsler by Dr. Irving Kron and Dr. D. Craig Miller, Chair of the Publication Committee and President respectively of the AATS, and a farewell message from Dr. Wechsler. All cardiothoracic surgeons everywhere owe Dr. Wechsler a debt of gratitude for providing such a high quality journal that teaches and guides us all.

But the JTCVS remains in good and capable hands, as Dr. Lawrence Cohn of Boston assumes the Editor's post with the January issue. Journal readers can be assured that the publication will flourish under his editorship.

New CTSNet Member Organization

CTSNet welcomes a new member organization, The Accreditation Committee - Perfusion Education (AC-PE). The (AC-PE) is a Committee on Accreditation (CoA) under the Commission on Accreditation of Allied Health Education Programs (CAAHEP). The mission of the AC-PE is to cooperate to establish, maintain and promote appropriate standards of quality for educational programs in perfusion and to provide recognition for education programs that meet or exceed the minimum standards outlined in the Standards and Guidelines for the Accreditation of Educational Programs in Perfusion. It is the responsibility of the AC-PE to review and evaluate perfusion programs to determine compliance with the Standards and then recommend an accreditation action to the Commission on Accreditation of Allied Health Education Programs (CCAHEP). CAAHEP actually makes the award of accreditation. To carry out its mission, the AC-PE evaluates new and currently accredited perfusion programs, conducts random site visits, and conducts approximately 2-4 board meetings per year.

Membership of AC-PE consists of two representatives appointed from each of the following seven (7) sponsors:

Perioperative Blood Management Technologist Exam

The American Society of ExtraCorporeal Technology is pleased to offer a special symposium on Techniques of Blood Management. This symposium will be offered on April 8, 2008, immediately preceding the AmSECT 46th International Conference in Orlando, FL. Subject areas to be presented include: Bloodless Surgery and the Improvement of Cardiopulmonary Bypass, Minimizing Bleeding and Allogeneic Blood Transfusion, The Role of Extracorporeal Interventions on Optimizing Perfusion, Heparin Induced Thrombocytopenia: Alternative Treatment Methods, Advances in Coagulation Monitoring: Present and the Future, Mechanisms of Reducing Inflammation during Cardiopulmonary Bypass, Autologous Platelet Harvesting and its Applications. Following the Symposium, the International Board of Blood Management (IBBM) will administer a certification exam for specialization in blood management techniques. The examination is a prerequisite to certification by the IBBM as a Perioperative Blood Management Technologist. This exam is available to individuals who are currently performing techniques of clinical autotransfusion and cell processing in the perioperative environment. In the December 2007 issue of The Annals, we have an interesting paper from Timone Hospital in Marseilles, France, dealing with surgical therapy of infectious endocarditis (IE). The authors note that the best type of prosthesis to be implanted during the active phase of endocarditis remains controversial.

Among 127 consecutive patients, mean age 57 ± 15 years, 30% with preexisting aortic prosthesis, and 63 (50%)^ with annulus abscess, 54 (43%) were treated with aortic homograft and 73 (57%) with a non-homograft prosthesis. Median time between diagnosis and surgery was 10 days. In-hospital mortality was 9%, not different between homograft and non-homograft prostheses (p = 0.6). By multivariable analysis, prosthetic valve endocarditis was the only variable independently associated with in-hospital mortality, which was not influenced by type of valvular substitute (/p/ = 0.6), even in the subset with annulus abscess (/p/ = 0.2). Ten-year survival free from the combined endpoint was 44% ± 10%, not different between homograft and conventional prostheses (log rank /p/ = 0.2). By multivariable analysis, comorbidity index and prosthetic valve endocarditis were independently predictive of the combined endpoint, which was not determined by type of valvular substitute (/p/ = 0.6) even in the subset with annulus abscess (/p/ = 0.5).

The authors concluded that implantation of conventional prostheses during the active phase of aortic endocarditis yields similar low operative mortality and long-term prognosis as compared with aortic homografts, even in patients with annulus abscess.

This result is at variance with the (often discussed) proposition that the homograft is superior in some way to other prosthetic valves, mechanical or xenograft. The issue of the Ross operation has not been specifically addressed in this study, but also figures prominently.

SELECTED ARTICLES OF INTEREST

In his final editorial, Dr. Andrew Wechsler "passes the torch" and reflects on his tenure as Editor of JTCVS. Dr. Antonio Corno and colleagues compare the efficacy of conventional pulmonary artery banding with that of FloWatch pulmonary artery banding and discuss why FloWatch pulmonary banding appears superior to conventional banding. Dr. Markl et al examine flow characteristics inside ventricular assist devices using magnetic resonance imaging techniques. Their article is enhanced by a number of color illustrations and an on-line video. Dr. Nakajima and associates evaluate the usefulness of endobronchial ultrasound-guided transbronchial needle aspiration to detect mediastinal and hilar lymph node metastasis in patients with potentially resectable metastatic lung tumors. These are just a few of the many topics covered in this month's issue.

37 patients with aortic stenosis and 39 with aortic insufficiency were followed for 2 years after operation. Geometric remodeling, regression of hypertrophy, and increased ejection fraction of the left ventricle were similar in both groups at 6 months, but after 2 years greater improvement was found in patients who had undergone valve replacement for aortic stenosis.

Novel Method of Thoracoscopic Surgery for Giant Bulla without Residual Cavity.

The roof of the bulla is resected first, followed by resection of the pulmonary parenchyma, including the base of the bulla, using a stapling device. Exposure of the base by first removing the roof facilitates determination of the resection line. Results were superior with this technique.

The Journal is published in Portuguese or English in the printed version, and in Portuguese and English in the online version. Full-text access is free. This is the only Journal in the cardiovascular surgery field recognized by MEDLINE/PUBMED in the Southern Hemisphere.

The American Society of ExtraCorporeal Technology is pleased to offer a special symposium on Techniques of Blood Management. This symposium will be offered on April 8, 2008, immediately preceding the AmSECT 46th International Conference in Orlando, FL. CITATION OF CTSNET SCHOLARLY PUBLICATIONS

We continue to get inquiries about citing articles published on CTSNet, so here is some information that should be useful. If you publish a scholarly paper on CTSNet, you should cite it in your CV. Style manuals and Index Medicus have been haggling about the proper format for several years, principally because, unlike print, net papers can be modified from time to time. Therefore, the citation should include the date of (a) posting or of modification, and/or (b) date of access when quoted as a reference. This assures that the proper iteration of the paper is being cited.

For your reference in your own CV:

As time passes, more and more net references will be found in CVs, and they will be considered seriously by promotion committees, just as print articles are now.
