CAN A TISSUE VALVE IMPACT PATIENT SURVIVAL?
Can aspects of valve performance--calcification, regurgitation, hemodynamics and implantability--impact patient survival? Although we can debate the reasons, the outcomes are real. For more information about Medtronic tissue valves, contact your local sales representative or call 1-800-633-8766 or 1-763-514-7107.

CG FUTURE® COMPOSITE RING: A UNIQUE COMPOSITE DESIGN FOR DYNAMIC MITRAL REPAIR
View our new CG Future® Composite Ring animation. Mitral annulus movement is closely related to aortic root dynamics--the correct function of one valve requires the integrity of the other. Introducing the first composite ring to offer posterior remodeling while maintaining anterior flexibility, restoring dynamic annular motion. Contact us at 1-877-526-7890 to see the COMPOSITE difference for yourself.

OTHER ARTICLES OF INTEREST
Dr. Bavaria and colleagues present the results from the first completed multicenter trial directed at gaining approval from the US Food and Drug Administration of endovascular versus open surgical repair of descending thoracic aortic aneurysms. Drs. Elefteriades and Perry put the results of the trial in perspective and urge that the enthusiasm for this new modality be mixed with a bit of skepticism or at least realism. In keeping with the educational mission of the Journal, Drs. Di Donato, et al., comment on previously published findings by Dr. O'Neill and colleagues concerning the residual high incidence of ventricular arrhythmias after left ventricular reconstructive surgery. Dr. O'Neill, et al, keep the debate alive by commenting further on the issues raised in Di Donato's editorial. Percutaneous aortic valve replacement is the topic debated in editorials by Drs. di Marco and Gerosa. Ethics Editor Martin McKneally looks at managing conflicts of interest in cardiothoracic surgery and explores the conscious and unconscious biases that can influence person perceptions.

Castiglioni A, et al. Minimally invasive closed circuit versus standard extracorporeal circulation for aortic valve replacement. Ann Thorac Surg 2007;83:586-91.
"The authors of this study evaluated the safety and clinical results of aortic valve replacement performed with minimally invasive closed circuit extracorporeal circulation technique (MECC system) compared to standard cardiopulmonary bypass. The MECC system (Maquet-Cardiopulmonary [Rastatt, Germany] is a low priming circuit without any blood-air interface.
Forty consecutive patients undergoing isolated aortic valve replacement were randomized to either miniaturized closed circuit cardiopulmonary bypass with the MECC system (group B, n = 17) or standard cardiopulmonary bypass (group A, n = 23). Technical feasibility, in particular the potential entry of air in the circuit, and clinical results were prospectively evaluated.
Demographic characteristics and surgical data were similar in both groups. Patients in the MECC group showed reduced chest tube drainage (217 +/- 62 mL vs 420 +/- 219 mL, p < 0.05) and blood transfusion requirements (5.1% vs 43.4%, p < 0.02) compared with patients in the control group. Moreover, the MECC group showed significantly higher time course of hematocrit at all time points during the operation and longer hospital stay (p < 0.02) than the control group. Similarly, in the MECC group patients' platelet count in intensive care unit admission was significantly higher than the control group (140 +/- 29 x 10(9)/L vs 119 +/- 37 x 10(9)/L, p < 0.05). Peak postoperative troponin C release was significantly lower in the MECC group (4.74 +/- 2.82 vs 8.43 +/- 6.25 ng/dL, p < 0.033). One patient in the MECC group showed a major neurologic event on postoperative day four, which was probably secondary to severe aortic calcification.
The authors concluded that the MECC system is suitable for aortic valve replacement and provides better clinical results than standard cardiopulmonary bypass as regards blood transfusion requirements, platelets consumption, and myocardial damage. This study has widespread implications for cardiac surgery, whether or not a minimally invasive approach is considered.
We invite comments regarding similar experience in other institutions, with MECC or similar bypass techniques. Please read the article and add your comments and experience to this Dilemma in Adult Cardiac Surgery in the CTSNet Discussion Forum."
