Applications for the 2007 Recertification process will be mailed to all eligible Diplomates in 2007. Eligible Diplomates are those individuals who certified in 1977, 1978, 1979, 1987, 1988, 1989, 1997, 1998, and 1999.
New Certification Pathways and Operative Requirements:
In response to current practice patterns in Thoracic Surgery and to insure adequate preparation for contemporary Thoracic Surgery practice, the American Board of Thoracic Surgery has revised the standards required of individuals to qualify for entrance into its certification process. Starting in July 2007, the Board will establish 2 primary pathways to certification, a Cardiothoracic Surgery Pathway and a General Thoracic Surgery Pathway. New Congenital Heart Surgery Certificate:
An additional special certificate pathway will be established for candidates who complete the Cardiothoracic Surgery Track and plan to perform congenital heart surgery. For more information on the new Certificate of Added Qualifications in Congenital Heart Surgery, Click Here.
Maintenance of Certification Initiative:
In response to an initiative by the American Board of Medical Specialties, the ABTS along with the other medical certifying boards has begun the transition towards a Maintenance of Certification Program® (MOC), to replace the current Recertification process. For more information on MOC, Click Here.

William M. Novick Nominated for Passion Award
William Novick, MD, Pediatric Cardiac Surgeon from the University of Tennessee, Memphis, TN, and Director of the International Children's Heart Foundation, has been nominated and short listed for the Passion Award. The award, given yearly by Frederique Constant of Geneva, recognizes a commitment to and success in one's field. William S. Pierce, MD, receives the ACS 2007 Jacobson Award
William S. Pierce, MD, thoracic surgeon and chemical engineer from Hershey, Pennsylvania, is the recipient of the 2007 Jacobson Innovation Award of the American College of Surgeons (ACS). Dr. Pierce was honored with the award in recognition of his pioneering work in the conception and development of mechanical circulatory support and the total artificial mechanical heart and his contributions to surgical bioengineering and patient care. His contributions to science, medicine, and education have benefited countless surgical patients with heart failure. Dr. Pierce is the Evan Pugh Emeritus Professor of Surgery at Pennsylvania State University College of Medicine, Hershey, PA.
The 13th Jacobson Innovation Award was presented to Dr. Pierce at a dinner on June 8, 2007, at the John B. Murphy Memorial Auditorium, Chicago, Illinois. The purpose of the award is to honor living surgeons who have been innovators of a new development or technique in any field of surgery. The award is made possible through a gift from Julius H. Jacobson II, MD, FACS, and his wife Joan. Dr. Jacobson is a general vascular surgeon known for his pioneering work in the development of microsurgery.

Advanced Endosurgical Training for Cardiac Surgery Teams Course
The course is designed for both cardiac surgeons and their surgical team members to include perfusionists, scrub techs, PAs and other OR personnel. The surgeons will participate in hands-on training via advanced simulation as well as procedural complication management and advanced endosurgical device study.
The surgery team member will be participating in a simultaneous track on the basics of endosurgery. The two tracks will then be brought together near the end of the course to work as a team on simulation. The simulation packages included for this course are Transapical Aortic, Retrograde Aortic and TAA.
*** This course is specifically design for the cardiac surgeon who has completed the EDGE Basic Guidewire, Catheter and Imaging Skills Course or has significant experience in endosurgical procedures as the primary operator. Basic Minimally Invasive Hands-on and Cadaver Training for Cardiac Surgeons
The course is designed for cardiac surgeons. Surgeons will operate on cadavers under the instruction of distinguished faculty. Surgeons will be trained on minimally invasive approaches to direct vision valve surgery and port-access surgical ablation. The surgeons will be exposed and will be able to try new instruments and techniques that will improve their skills and capabilities as minimally invasive surgeons. This month CTSNet reintroduces survey tools on the Thoracic Portal. Surveys were initially introduced on the Thoracic Portal in August 2006, and surveys have been used sporadically on the CTSNet home page since its inception. However, we have found that, although results have been interesting and potentially informative, the response rate hasn't been sufficiently high to permit use of these surveys to importantly influence thought or behavior.
Surveys, although used informally in our setting, have substantial potential value to the leadership of CTSNet. Our initial survey asked members about what they like on the Thoracic Portal and what changes might be desirable. This information, combined with data regarding page reads for various content on the portal, provides useful insight into how we can tailor the content of the portal to the needs of the readership. Our aim is to be as responsive to CTSNet members as possible, providing them with the information that they need to optimize their professional lives.
Surveys also can benefit CTSNet members in more immediate clinical ways. Thus far we have posted results of four clinical surveys, and a fifth clinical survey is available for members to complete. The four completed surveys focus on clinical management of lung and esophageal cancer. The current survey explores management of primary spontaneous pneumothorax. In each of the surveys published to date we found surprising variations in management of common clinical problems. Such results are bound to give the readership insight into alternative methods for approaching conditions that they may see nearly every day.
In addition to learning results of survey questions posed by others, the survey tool offers the CTSNet readership the opportunity to develop surveys of their own. Perhaps you, the reader, are curious about how different surgeons might manage a specific clinical entity that you and others have found challenging. Simply provide a clinical scenario and suggested survey questions that address the issue, and the survey will be considered for posting during one of our updates.
For the future, surveys will be posted every four to six weeks. The posting of a new survey will be announced on the CTSNet home page and on the Thoracic Portal main page. The survey tool link will be highlighted with a flashing reminder on the Thoracic Portal main page. After each survey is completed, the results will be tabulated and posted within two weeks along with a brief summary and commentary to help contextualize the outcomes.
The results of competed surveys to date can be found in the right gutter of the Thoracic Portal Home Page.
This updated survey content is designed to provide additional value to your CTSNet membership. As always, we welcome comments and suggestions regarding it and all elements of the Thoracic Portal experience.

CTSNET IMAGE LIBRARY: The explosion in digital images and video in the past decade has provided a great opportunity for cardiothoracic surgeons to create and share spectacular images of surgical techniques. CTSNet is seeking contributions to build an online Image Library. The library will be a collection of still and videos of the highest quality that comprehensive illustrate cardiothoracic surgery. CTSNet invites contributions from all professionals in cardiothoracic surgery who wish to make images and videos available for educational use by the public. CTSNet will request a nonexclusive, royalty-free license to publish and distribute images through the CTSNet Web site. Contributors will maintain copyright and will be permitted to use their contributions elsewhere. Vascular Surgical Society International Traveling Fellowship
The Edward B. Diethrich Vascular Surgical Society (EBDVSS) International Traveling Fellowship was established to broaden the educational experience of a cardiac or vascular surgical resident interested in endovascular surgery during their final year of training by providing an opportunity for travel to one or more established Centers of Excellence in the applicant's field of interest. One award will be granted to a North American resident to support research and/or enhancement of endovascular skills at a host institution abroad in hopes of stimulating academic inspiration, promoting international exchange and fostering fraternal fellowship. A second award will be granted to an International (non-North American) resident for travel to a North American Center of Excellence
The grant of $10,000 will be provided to the successful applicants to underwrite their travel and living expenses incurred during a maximum of three weeks of training at one or more Endovascular Centers of Excellence. The funds are also intended to provide for roundtrip travel and accommodations at the International Congress XXI on Endovascular Interventions scheduled for February 10-14, 2008 in Scottsdale, AZ. Registration for the Congress and all related events will be covered by the EBDVSS. Fellowship travel must be completed prior to January 1, 2008.
Applicants must have approval from their home and prospective host institutions. The application consists of a one-page proposal summarizing what the applicant hopes to accomplish during their training, a current curriculum vitae and a cover letter outlining their career goals. Additionally, award recipients must agree to present a summary of their experience at the International Congress.
THE DEADLINE FOR APPLICATIONS IS AUGUST 7, 2007. Notification of successful applicants will take place no later than September 1, 2007.

Lam, et al, consider the impact of patient-prosthesis mismatch on late outcomes after mitral valve replacement. Pibarot and Dumesnil, in a timely editorial, define prosthesis-patient mismatch and discuss why it is important and how to prevent it. Tardif, et al, evaluate the potential cardioprotective and neuroprotective effects of pyridoxal-5-phosphate in patients undergoing high-risk coronary artery bypass surgery (the MEND-CABG study). Edward Verrier, in an accompanying editorial, emphasizes the importance of looking beyond the technology to understand the science involved in this complex clinical challenge. Lederer, et al, discuss the improvement in body mass index, airflow obstruction, dyspnea, and exercise capacity (BODE) index after 1 year in patients with pulmonary emphysema who underwent lung-volume reduction surgery. Keith Naunheim puts the results in perspective by looking at the advantages and limitations of applying the BODE index to predicting long-term outcomes in these patients.

ANNALS OF THORACIC SURGERY DISCUSSION PAPER - JUNE 2007
In the June 2007 issue of The Annals, Zhang and associates report their unique experience with a new type of valved VSD patch, and the abstract is excerpted herein. Severe pulmonary hypertension is a common complication of congenital cardiac defects with large left to right shunt, and the closure of a large ventricular septal defect (VSD) with elevated pulmonary vascular resistance (PVR) is associated with significant morbidity and mortality. A unidirectional monovalve homologous aortic patch was designed to close the large VSD with severe pulmonary hypertension in an effort to decrease the morbidity and mortality. Twenty-seven patients (mean age, 15.0 +/- 5.6 years) with large VSD with severe pulmonary hypertension (pulmonary vascular resistance, 15.2 +/- 3.8 Wood units) were repaired with this technique. According to body surface area and the preoperative arterial oxygen saturation, the monovalve homologous aortic patches were fenestrated on the aortic wall with a diameter of 4 to 8 mm. Two patients died of pulmonary hypertensive crisis and cardiac arrest postoperatively. At 5 months to 10 years follow-up, all of the surviving patients were improved with no late death. Obvious opening and closing of the monovalve was detected by early postoperative echocardiography in seven patients. A small amount of right to left shunt was detected in three patients three months after operation, and in two of them the shunt still existed three years after operation. The authors conclude that closure of a large VSD in patients with severe pulmonary hypertension could be performed with low morbidity and mortality when a unidirectional monovalve homologous aortic patch was used and the long-term result was satisfactory.
This technique is novel, although other types of valve patches have been used in the past. The important point is that the results may challenge our traditional beliefs about operability of VSD patients in the current era.
