ATS MEDICAL ACQUIRES 3F THERAPEUTICS
ATS Medical, Inc., developer, manufacturer and marketer of state-of-the-art cardiac surgery products and services, announced a definitive agreement under which ATS will acquire all the outstanding shares of privately-held 3F Therapeutics in a stock for stock transaction. The acquisition of 3F Therapeutics is viewed as a major step in executing ATS Medical’s longstanding vision of obtaining a leadership position in all segments of the cardiac surgery market.
3F Therapeutics was founded in 1998 with a focus similar to that of ATS Medical, on serving the needs of the cardiac surgeon. 3F Therapeutics’ fundamentally unique heart valve product design concepts are intended to improve on the performance of existing heart valves by mimicking natural valves. With the belief that there is not a single solution for each heart valve implant, 3F Therapeutics has worked toward offering cardiac surgeons several biological tissue heart valve options from which to choose. Its first product, the 3F Aortic Bioprosthesis™, has been commercially released in Europe.

EDWARDS ANNOUNCES LAUNCH OF NEW MITRAL VALVE REPAIR THERAPY
Edwards Lifesciences Corporation announces the European and U.S. launch of a new mitral valve repair therapy designed to treat mitral valve insufficiency in patients suffering from congestive heart failure.
The Edwards GeoForm annuloplasty ring, which features a unique, three-dimensional shape, has been shown to treat successfully mitral regurgitation caused by cardiomyopathy and to improve the function of the left ventricle, according to a presentation delivered at the Annual Meeting of the European Association for Cardio-thoracic Surgery (EACTS) in Barcelona. Cardiomyopathy is a common form of congestive heart failure that affects tens of thousands of people annually.
In his presentation, "GeoForm: A New Valvular Solution for a Ventricular Problem," Steven F. Bolling, MD, Professor of Surgery and Director of the University of Michigan Health System's Mitral Valve Clinic, presented results from a prospective study of 16 congestive heart failure patients, all of whom were suffering from mitral valve regurgitation due to distortion of the left ventricle. Six months after the patients' mitral valves were surgically repaired with the Edwards GeoForm annuloplasty ring, their mitral valve regurgitation was eliminated and the left ventricle was reformed.
Edwards Lifesciences developed the GeoForm ring with Prof. Ottavio M. Alfieri, MD, of St. Raffaele Hospital in Milan, Italy and with Dr. Bolling.

A guest speaker of the organization, Dr. Timothy Gardner, had this to say:
"The annual meeting of the Society for Cardiothoracic Surgeons in Great Britain and Ireland, held this year in Dublin, was a model for a really good surgical meeting. Those in attendance spanned the spectrum from senior surgeons and professors through actively practicing surgeons and a few anesthesiologists, plus senior registrars and trainees, to cardiothoracic advanced practice nurses, database managers, perfusionists, and other allied professionals. There were even local medical students who assisted with the interactive features of the program. The Program itself had lots of new science, clinically relevant presentations, competition of basic and clinical research presentations from registrars and trainees, and even a few spirited debates. What distinguished this meeting was the high degree of inter-activity among presenters, invited discussants and moderators, and the very alert audience. In addition, the social events were well attended, easygoing, and very engaging, which resulted in lots of networking and social exchange. The size of the meeting and the ease of interaction, including with the many exhibitors, resulted in a very satisfying professional and social experience.”

MARCH 2006 ARTICLE FOR DISCUSSION
Michielon G, et al. Genetic syndromes and outcome after surgical correction of tetralogy of Fallot.
The impact of genetic syndromes on results of surgical repair of TOF in infancy was investigated and reported by the experienced Bambino Gesu team. 306 consecutive patients (median age, 5.1 months) underwent primary (266) or staged (40) repair of TOF between 1994 and 2004. Total follow-up was 1,188 patient-years (mean, 57 months). Genetic syndromes were documented in 85 patients (27.8%), including 22q11 deletion (27), trisomy 21 (13), vertebral, anal, cardiac, tracheoesophageal, renal, and limb abnormalities (VACTERL, 12), and others (33). Hypoplastic pulmonary arteries (PA) were more common in syndromic (19/85 = 22.3%) than nonsyndromic TOF (20/221 = 9.04%) (p < 0.001). Primary repair was performed in 82.4% syndromic and 88.7% nonsyndromic TOF. Ten-year actuarial survival was 94.1 +/- 2.3% in nonsyndromic and 84.3 +/-4.2% in syndromic TOF (p < 0.001). Cox proportional hazard identified the presence of a genetic syndrome (p = 0.011) and central PA hypoplasia (p = 0.002) as independent predictors of mortality. The authors concluded that pulmonary arborization defects and genetic syndromes other than del22q11 or trisomy 21, were associated with worse outcome after correction of TOF. They propose that primary TOF repair in syndromic patients with normal PA anatomy is a valid surgical strategy, with no additional risk for mortality and higher freedom from reintervention. What to do about the patients who are syndromic and have abnormal PA anatomy remains an issue. The policy for such patients still varies considerably from unit to unit, and the issues surrounding palliation morbidity and mortality remain important. This paper therefore is both important and controversial.

ARTICLES OF INTEREST IN THIS MONTH’S ISSUE:
Dr. Martin McKneally, Ethics Editor, tackles two difficult questions: how many authors can reasonably be listed on a published paper and what qualifies a contributor to be an author. He sketches a conceptual framework and makes some useful suggestions.
Dr. Cornelius Dyke and colleagues compare bivalirudin to heparin in patients undergoing cardiac surgery with cardiopulmonary bypass (the EVOLUTION-ON study). In a related study, also from the EVOLUTION-ON study, Dr. Nicholas Smedira and colleagues evaluate the safety and efficacy of bivalirudin in patients undergoing off-pump coronary artery bypass grafting. Dr. Theodore Warkentin, in an accompanying editorial, evaluates the results of both studies.
Dr. Francis Spinale and colleagues report on results of pretreatment with statins before cardiac surgery. Dr. Harold Lazar, in an accompanying editorial, looks at the limitations of the study andputs the results in perspective.
Dr. A. J. Basmadjian and coworkers investigate whether a good perioperative echocardiographic result is predictive of durability in ischemic mitral valve repair. Drs. David H. Adams and Anelechi Anyanwu discuss the larger picture, providing a detailed analysis of the current environment of the clinical practice of valve repair.
Interested readers are urged to look at the STS/AATS Position Statement position statement, published simultaneously in JTCVS and The Annals of Thoracic Surgery, Guidelines for Credentialing of Practitioners to Perform Endovascular Stent-Grafting of the Thoracic Aorta.

The Annual Meeting of the German Society for Thoracic and Cardiovascular Surgery recently took place in Hamburg. The scientific quality of presentations was much appreciated by a record number of attendees. “Wind of Change,” which was the motto for the meeting, was certainly fulfilled in all aspects. For the first time in the 35-year history of the Society a visiting country was invited, Japan. Several Japanese delegates were welcomed to the meeting adding a nice international flavor and interesting and innovative scientific exchange.”
