CTSNet Announces Search for Editor of the Residents Section
Since inception, the Residents Section has been under the supervision by a CT resident in training, the idea being that the residents themselves know best what material they wish to have on their site. There have been three editors to the present time, Dr. John Liddicoat, Dr. John Doty, and the current editor, Dr. Vinod Thourani. Dr. Thorani was kind enough to serve an extra year after he finished his resident training, but it is time for him to move on.
Therefore, we are announcing an open search for a Resident Section Editor.
Criteria for consideration for the position are as follows:
(1) Must be a current resident in cardiothoracic surgery, or beginning residency in July, 2007, and remain so for at least two years (July 2007 - June 2009)
(2) Must have working knowledge of CTSNet, as well as the global net environment.
(3) Must have access to a broadband Internet service.
(4) Responsibilities will include maintenance and oversight of the entire Resident's section; official representation of CTSNet to the Thoracic Surgery Resident's Association (TSRA); collaboration with other resident organizations around the world.
(5) The applicant must have the approval and support of his program director for this activity.

Vascular Surgical Society International Traveling Fellowship
The Edward B. Diethrich Vascular Surgical Society (EBDVSS) International Traveling Fellowship was established to broaden the educational experience of a cardiac or vascular surgical resident interested in endovascular surgery during their final year of training by providing an opportunity for travel to one or more established Centers of Excellence in the applicant's field of interest. One award will be granted to a North American resident to support research and/or enhancement of endovascular skills at a host institution abroad in hopes of stimulating academic inspiration, promoting international exchange and fostering fraternal fellowship. A second award will be granted to an International (non-North American) resident for travel to a North American Center of Excellence
The grant of $10,000 will be provided to the successful applicants to underwrite their travel and living expenses incurred during a maximum of three weeks of training at one or more Endovascular Centers of Excellence. The funds are also intended to provide for roundtrip travel and accommodations at the International Congress XXI on Endovascular Interventions scheduled for February 10-14, 2008 in Scottsdale, AZ. Registration for the Congress and all related events will be covered by the EBDVSS. Fellowship travel must be completed prior to January 1, 2008.
Applicants must have approval from their home and prospective host institutions. The application consists of a one-page proposal summarizing what the applicant hopes to accomplish during their training, a current curriculum vitae and a cover letter outlining their career goals. Additionally, award recipients must agree to present a summary of their experience at the International Congress.
THE DEADLINE FOR APPLICATIONS IS AUGUST 7, 2007. Notification of successful applicants will take place no later than September 1, 2007.

EACTS Establishes Techno-College Innovation Award
The Techno-College invites surgeons and individuals from companies active in the field of Thoracic and Cardiovascular Surgery to apply for The Techno-College Innovation Award.
We are looking for technological breakthroughs in all areas related to thoracic and cardiovascular research in particular for new surgical methods or devices. Innovations can be in the form of patents, inventions, new products, ideas or services. The innovations should have the potential to change our standard practice and should go beyond marginal improvements in existing procedures or products.
The winner will be chosen on behalf of the European Association of Cardio-Thoracic Surgery (EACTS) by the members of the New Technology Resource Group. The winner will have the opportunity to present his/her work during the Annual Techno-College on Saturday September 15 2007. The prize, Euro 5,000, will be awarded during the EACTS Presidential Dinner on Tuesday September 17 2007. PLEASE COMPLETE AND SUBMIT THIS FORM WITH ANY SUPPORTING DOCUMENTATION BY JULY 1, 2007.

Announcement from The Thoracic Surgery Residents Association (TSRA)
The Thoracic Surgery Residents Association (TSRA) is announcing nine (9) open positions on the Executive Committee.
Each executive committee member is also assigned a position as the TSRA representative within a national organization including the Residency Review Committee, AATS, STS, and Joint Council for Thoracic Surgery Education. The TSRA representative must attend the national meetings for his or her respective organization and be an active participant.
These positions require dedication and work. In order to accurately represent the thoracic residents, each executive member must participate in activities to communicate with residents (surveys, emails, etc), so that we speak in one voice, not offering a smattering of opinions.
You may apply if you are starting in a CT residency July 07.
You must still be a resident through June 30, 2008 to be considered.

New Book Featured in CTSNet Bookstore

This month CTSNet reintroduces survey tools on the Thoracic Portal. Surveys were initially introduced on the Thoracic Portal in August 2006, and surveys have been used sporadically on the CTSNet home page since its inception. However, we have found that, although results have been interesting and potentially informative, the response rate hasn't been sufficiently high to permit use of these surveys to importantly influence thought or behavior.
Surveys, although used informally in our setting, have substantial potential value to the leadership of CTSNet. Our initial survey asked members about what they like on the Thoracic Portal and what changes might be desirable. This information, combined with data regarding page reads for various content on the portal, provides useful insight into how we can tailor the content of the portal to the needs of the readership. Our aim is to be as responsive to CTSNet members as possible, providing them with the information that they need to optimize their professional lives.
Surveys also can benefit CTSNet members in more immediate clinical ways. Thus far we have posted results of four clinical surveys, and a fifth clinical survey is available for members to complete. The four completed surveys focus on clinical management of lung and esophageal cancer. The current survey explores management of primary spontaneous pneumothorax. In each of the surveys published to date we found surprising variations in management of common clinical problems. Such results are bound to give the readership insight into alternative methods for approaching conditions that they may see nearly every day.
In addition to learning results of survey questions posed by others, the survey tool offers the CTSNet readership the opportunity to develop surveys of their own. Perhaps you, the reader, are curious about how different surgeons might manage a specific clinical entity that you and others have found challenging. Simply provide a clinical scenario and suggested survey questions that address the issue, and the survey will be considered for posting during one of our updates.
For the future, surveys will be posted every four to six weeks. The posting of a new survey will be announced on the CTSNet home page and on the Thoracic Portal main page. The survey tool link will be highlighted with a flashing reminder on the Thoracic Portal main page. After each survey is completed, the results will be tabulated and posted within two weeks along with a brief summary and commentary to help contextualize the outcomes.
The results of competed surveys to date can be found in the right gutter of the Thoracic Portal Home Page.
This updated survey content is designed to provide additional value to your CTSNet membership. As always, we welcome comments and suggestions regarding it and all elements of the Thoracic Portal experience.

Advanced Endosurgical Training for Cardiac Surgery Teams Course
The course is designed for both cardiac surgeons and their surgical team members to include perfusionists, scrub techs, PAs and other OR personnel. The surgeons will participate in hands-on training via advanced simulation as well as procedural complication management and advanced endosurgical device study.
The surgery team member will be participating in a simultaneous track on the basics of endosurgery. The two tracks will then be brought together near the end of the course to work as a team on simulation. The simulation packages included for this course are Transapical Aortic, Retrograde Aortic and TAA.
*** This course is specifically design for the cardiac surgeon who has completed the EDGE Basic Guidewire, Catheter and Imaging Skills Course or has significant experience in endosurgical procedures as the primary operator. Basic Minimally Invasive Hands-on and Cadaver Training for Cardiac Surgeons
The course is designed for cardiac surgeons. Surgeons will operate on cadavers under the instruction of distinguished faculty. Surgeons will be trained on minimally invasive approaches to direct vision valve surgery and port-access surgical ablation. The surgeons will be exposed and will be able to try new instruments and techniques that will improve their skills and capabilities as minimally invasive surgeons. In 2005, Dr. David, working with engineers from Medtronic, designed the Simplici-T® Annuloplasty System for use in mitral and tricuspid valve repair surgery to help restore the leaking valve opening to a more normal shape. The Simplici-T band restores proper valve coaptation and improves valve function after repair.

New Feature - CTSNET IMAGE LIBRARY
CTSNET IMAGE LIBRARY: The explosion in digital images and video in the past decade has provided a great opportunity for cardiothoracic surgeons to create and share spectacular images of surgical techniques. CTSNet is seeking contributions to build an online Image Library. The library will be a collection of still and videos of the highest quality that comprehensive illustrate cardiothoracic surgery. CTSNet invites contributions from all professionals in cardiothoracic surgery who wish to make images and videos available for educational use by the public. CTSNet will request a nonexclusive, royalty-free license to publish and distribute images through the CTSNet Web site. Contributors will maintain copyright and will be permitted to use their contributions elsewhere. Individual images and videos should be under 10 megabytes, and users can upload up to 100 megabytes per session. Individual user albums have a 1 gigabyte storage limit. Norman E. Shumway Research Scholarship (2008-2010) provides an opportunity for research, training and experience for North American surgeons committed to pursuing an academic career in cardiothoracic surgery.
Research program must be undertaken within the first three years after completion of an approved North American cardiothoracic residency. Applications for the scholarship may be submitted during the candidate's final year of cardiothoracic residency or during his or her first two years in an academic position. The duration of the scholarship is to be two years.

In the May 2007 Annals, Sagbas and colleagues from Istanbul evaluate the feasibility and the efficacy of a new off-pump, bilateral thoracoscopic pulmonary vein isolation technique in patients with atrial fibrillation. Twenty-sis (26) drug-resistant and symptomatic isolated atrial fibrillation patients (18 permanent, 8 paroxysmal, mean age 55 years) underwent an irrigated radiofrequency ablation procedure using the Cardioblate ablation system (Medtronic). Mean duration of atrial fibrillation was 34.2 ± 18.9 months. All patients underwent a bilateral thoracoscopic procedure in which both pulmonary veins were ablated with an atrial cuff using an off-pump epicardial approach. The conduction block was assessed by pacing the pulmonary veins after each ablation. Sixteen patients underwent endoscopic stapling of the left atrial appendage. There was no mortality and all procedures were completed as planned without any conversions to sternotomy. There were no major complications. Follow-up was complete at 6 months, and 80% of the patients were in sinus rhythm (paroxysmal: 100%, permanent: 72%). Of the patients with permanent atrial fibrillation, 85% had regained their atrial transport function. No major thromboembolic event was observed during the follow-up period.
The authors concluded that the video-assisted bilateral pulmonary vein isolation technique was safe and effective, curative for paroxysmal atrial fibrillation patients and effective for permanent atrial fibrillation cases. They also state that this technique may find wider application if accumulating data further support their findings.
This is certainly an area of active controversy, marked by competing technologies and approaches with comparable results. There is also the issue of follow-up time and adequacy of atrial isolation, as has been pointed out by Cox and others well experienced in other surgical approaches employing more extensive Maze operations. Gurbuz and colleagues at the Department of Cardiothoracic Surgery, Tucson Medical Center, Tucson, Arizona, USA, did a study of radial artery grafts in the setting of Off-pump CABG, and found that radial artery grafting was an independent predictor of increased symptom recurrence and adverse cardiac events. Patients with radial artery grafts also had a tendency towards more angina recurrence, coronary re-intervention, and sudden cardiac death when compared to patients with other types of conduits.
