Savage, St. John's Mercy Heart Center, St. Louis, Missouri USA. The following is a statement By Dr. Savage on the occasion of the Portal launch:
Announcement of CTS Portal
To the members of the CTSNet community: I would like to introduce the Cardiac Surgery Portal. You will see a link to this site in the left navigational menu on the main page, and if you choose, you may make the Portal your home page. The top of the Portal will have streaming announcements, similar to those on the main page, and it will have direct access to many of the content areas relevant to adult cardiac surgery, with highlights of new additions as they are posted. In addition, the Portal creates a framework for us to further develop content in an organized fashion. We hope that you find it useful, and we appreciate suggestions from you, as community members, regarding improvements that we can make to the Portal. The purpose of the Portal is to assist you in navigating through the content continually being added to CTSNet.

In his In My Opinion Article, Dr. Takamoto discusses "Guidelines for Live Surgery". Troubled by the sometimes cavalier attitude taken (or perceived) when operative telecasts are performed, members of three leading Japanese organizations developed a set of guidelines designed to control the more dangerous aspects of these teaching activities. These guidelines are now being studied by cardiothoracic surgical organizations around the world.
Dr. Shinichi Takamoto is Professor of Cardio-Thoracic Surgery, The University of Tokyo, Tokyo Japan. He is current Past President of the Japanese Association for Thoracic Surgery (new member of CTSNet), and Secretary-General of the Asian Society for Cardiovascular and Thoracic Surgery (ASCVS).

"The latest survey on CTSNet content
in the Thoracic Portal provides important information as to what users
find effective and what new content they would like to see. Clinical practice is most influenced by Experts' Techniques and by
clinical videos. Clinical knowledge is most influenced by current
literature. New features that users would most like to see
include difficult case discussions by experts, and critical analysis of
selected issues, either through pro-con debates or evidence-based
reviews. There is also some interest in availability of CME
credits on CTSNet. The CTSNet management group will be meeting in
the next few months to address these issues and will strive to make the
Thoracic Portal increasingly responsive to the users' needs."

ATS NOV 2007: Versaci F, et al. Sequential hybrid carotid and coronary artery revascularization: immediate and mid-term results. Ann Thorac Surg 2007;84:1508-14.
In the November 2007 issue of The Annals, Versaci et al (from Rome) discuss a novel therapeutic strategy consisting of simultaneous hybrid revascularization by carotid artery stenting, immediately followed by coronary artery bypass grafting. The study included 37 patients with severe carotid and coronary artery disease and a European System for Cardiac Operative Risk Evaluation (EuroSCORE) of 5 or higher. Immediately after CAS, patients underwent CABG. The primary end point was the incidence of stroke, myocardial infarction, or death at 30 days. Secondary end points were a combination of transient ischemic attack, major local complications, bleeding, and systemic complications within the 30 days after treatment, and any stroke, acute myocardial infarction, or death from day 31 through to the end of the follow-up.
The rate of procedural success was 97.3%, and the 30-day cumulative incidence of disabling stroke, myocardial infarction, or death was 8.1%. Two patients (5.4%) died, and 1 patient had a stroke immediately after carotid stenting. Another patient died between day 31 and 6 months after intervention.
The authors concluded that in elevated-surgical-risk patients with carotid stenosis and coronary artery disease suitable for CABG, hybrid revascularization by CAS, immediately followed by CABG, is a feasible and promising therapeutic strategy. This article is certain to generate controversy, as the best non-interventional approach is still a topic of discussion, and this article introduces a new strategy.
Please read the article and add your comments and experience to this Dilemma in Adult Cardiac Surgery in the CTSNet Discussion Forum.
The monthly articles for discussion are selected and summarized by Dr. Tom Karl, University of California at San Francisco, UCSF Children's Hospital, Department of Pediatric Cardiac Surgery, San Francisco, California USA. Dr. Karl is Editor of the ATS Discussion Section.

Michael K. Banbury, M.D. Named to Chair of Surgery
The W. Samuel Carpenter III Distinguished Chair of Cardiovascular Surgery, first held by Gerald M. Lemole, M.D., has been formally conferred upon Christiana Care Chief of Cardiac Surgery Michael K. Banbury, M.D.
Dr. Banbury completed his cardiothoracic surgery training at the Cleveland Clinic Foundation in 1997 and had a distinguished career there as a clinician and researcher for nine years before joining the Center for Heart & Vascular Health as Chief of Cardiac Surgery in April 2006. Dr. Lemole is the newly appointed medical director of Christiana Care's Eugene du Pont Preventive Medicine & Rehabilitation Institute and founder of the new Center for Integrative Health.
Dr. Lemole brought open-heart surgery to Delaware in 1986 after the opening of Christiana Hospital. He was the first person to be named to the chair, which was endowed in 2004 by a generous gift from the Good Samaritan Foundation in Mr. Carpenter's name.
The chair was established to help secure Christiana Care's continued leadership in the field of cardiovascular surgery and as a testament to Dr. Lemole's vision, leadership and exceptional contributions to the advancement of heart surgery in our community.
"It is an honor, a privilege and a welcome challenge to take responsibility to be continuing the great tradition of clinical excellence in cardiac surgery started by Dr. Lemole. The spirit of this tradition is embodied in the W. Samuel Carpenter III Distinguished Chair of Cardiovascular Surgery," Dr. Banbury says.
"I thank the Carpenter family for their confidence in me, my colleagues and Christiana Care," Dr. Lemole says. "It has been an honor to have served as the first W. Samuel Carpenter III Distinguished Chair of Cardiovascular Surgery. I believe the program will continue to grow and develop under the able leadership of the new Chairman, Michael Banbury, M.D."
Christiana Care received the 2007 Cardiac Surgery Excellence Award from HealthGrades, the nation's leading provider of health care quality measurements, while placing in the top 10 percent in the U.S. in cardiac surgery for four years in a row.

CITATION OF CTSNET SCHOLARLY PUBLICATIONS
We continue to get inquiries about citing articles published on CTSNet, so here is some information that should be useful. If you publish a scholarly paper on CTSNet, you should cite it in your CV. Style manuals and Index Medicus have been haggling about the proper format for several years, principally because, unlike print, net papers can be modified from time to time. Therefore, the citation should include the date of (a) posting or of modification, and/or (b) date of access when quoted as a reference. This assures that the proper iteration of the paper is being cited.
For your reference in your own CV:
Tripathy U, Enestvedt K, Komanapalli CB, Sukumar MS. Necrotizing esophagitis: the black esophagus. Available at CTSNet, Inc. http://www.ctsnet.org/sections/clinicalresources/clinicalcases/article-7.html. Posted 7 November 2005.
For an author quoting your reference in the bibliography of his or her paper:
Tripathy U, Enestvedt K, Komanapalli CB, Sukumar MS. Necrotizing esophagitis: the black esophagus. Available at CTSNet, Inc. http://www.ctsnet.org/sections/clinicalresources/clinicalcases/article-7.html. Posted 7 November 2005. Accessed 30 November 2005.
As time passes, more and more net references will be found in CVs, and they will be considered seriously by promotion committees, just as print articles are now.
MEMBER PHOTOS
We thank all those members who have sent your photos for posting on your home page. It does make such a difference when curious fellow-members can eyeball you. For those who have not yet done so, we urge you to send a photograph soon. We prefer a jpeg file attached to an e-mail, but will accept other types (bitmap, gif). Or simply send us a wallet-sized glossy print to the address below.
Upcoming Meetings
If you are planning to attend the Congress of the Association of Thoracic & Cardiovascular Surgeons of Asia November 25-28, 2007 in Nusa Dua, Bali, Indonesia, look for us in Booth #4 to say hello, learn more about CTSNet, and get your picture taken for your home page.
We wish you only the best during this holiday season, and a healthy, happy new year!
