Google Calendar
CTSNet has launched a new calendar feature using the Google web calendar to graphically display 2008 and 2009 events for all organizations. This supplements the CTSNet Event list, but provides additional help and convenience to the reader in finding information on a particular meeting. After you have located the dates of the meeting on the calendar, clicking on the name of the meeting at the top of the calendar day will display more information. Give it a try!

Readers comments
A new interactive feature has been put in place so that CTSNet readers may comment on an individual contribution directly without going to a discussion forum to do so. At present, this feature is showcased in two places to illustrate two types of access:
(1) a cardiac expert technique article: The reader comments material is found at the bottom of the right gutter information. This will be the location for most of the content on the website. Clicking in the box will permit you to view or add comments, although for the latter you will need your ID and password. New entries will be visible within 2 minutes from time of publication on the article page.
(2) A second type of comment access is found in the Safety Reporting System section, where under each vignette name is a comment link that will access the same system detailed in (1) above.
Any organizations that would like to use the Reader Comments system for their articles should contact Robert Oberteuffer (roberte@ctsnet.org) for setup.
Going forward, each new piece of scientific content posted on CTSNet will have this comment capability available. Because of its simplicity and convenience of use, we anticipate a lot of author/reader and reader/reader interactions, an activity that should be useful and instructive to everyone. Michael E. DeBakey
It is with heavy heart that CTSNet notes the passing of one of the greatest surgeons of all time. Michael E. DeBakey. He died quietly at 9:38 p.m. Friday, July 11 from natural causes at The Methodist Hospital in Houston, Texas, just short of his hundredth birthday. In a professional career that spanned 70 years he managed to pack several lifetimes of work. He will be missed the world over, by physicians and patients alike. His long-time colleague, Dr Gerald M. Lawrie, will write a tribute to the life of Dr. DeBakey for CTSNet.

Cardiothoracic Surgical Trials Network (CTSN) Call for Protocol Concepts
In recognition of the need for clinical evaluations in cardiac surgery the National Institutes of Health (through the National Heart, Lung and Blood Institute with collaboration from the National Institute of Neurological Disorders and Stroke) and the Canadian Institutes of Health Research have recently established the Cardiothoracic Surgical Trials Network (CTSN). The CTSN is a cooperative network of academic medical centers whose goal is to foster rigorous scientific comparisons and promote the evaluation of newer surgical techniques, technologies, devices, and innovative pharmaceutical and bioengineered products directed at cardiovascular disease. The CTSN is comprised of a Data Coordinating Center, The International Center for Health Outcomes and Innovation Research (InCHOIR) at Columbia University and eight Clinical Centers

Management of pericardial effusion
The following information was given at the beginning of this survey:
"For purposes of this survey, readers were asked to assume that they were consulted on a hospitalized middle aged patient who has been diagnosed with stage IV non-small cell lung cancer. The patient has shortness of breath in the absence of an important pleural effusion. There is no endobronchial obstruction and there is no evidence for pulmonary embolism. A CT scan demonstrates a pericardial effusion. Although echocardiography demonstrates a large pericardial effusion with no evidence for tamponade, there is some mild diastolic collapse."

The 17th European Conference on General Thoracic Surgery will be held in Krakow, Poland from the 31 May – 3 June 2009.

Residents are encouraged to submit abstracts for the 12th Annual C. Walton Lillehei Residents’ Forum.
The Forum will take place during the Annual Meeting on Sunday afternoon, May 10, 2009 from 3:00 p.m. – 5:00 p.m. at the Hynes Convention Center in Boston, MA. It will consist of presentations of original work by North American thoracic surgery residents and residents in general surgical training programs who are working in a cardiothoracic surgical laboratory or clinical rotation. The presentations are selected by the AATS Program Committee and the Training Program Director must acknowledge that the material being presented is original work and that of the resident.
Abstracts for the Lillehei Residents’ Forum must be received by October 6, 2008 by 11:59 p.m. Eastern Daylight Time.

Cardiac
In 2005, the American Heart Association and the European Resuscitation Council published a new comprehensive guideline for the management of patients who suffered a cardiac arrest. The guidelines were derived from a worldwide consensus from the International Liaison Committee on Resuscitation published that same year.
While the document provides significant advances in the resuscitation of out-of-hospital and ward-based patients, it does not address many of the issues that we face in cardiac surgery when a patient arrests shortly after surgery.
Issues such as the timing of chest reopening, CPR versus immediate defibrillation for VF, a 3-shock or a 1-shock protocol, Precordial thump, external pacing, administration of adrenaline in an arrest, sterility of chest reopenings, personnel, and training for cardiac arrests are unaddressed.
We would very much appreciate your input on these issues and encourage you to participate in a survey. The responses collected in the survey will be used in drafting a more comprehensive set of guidelines for resuscitation of patients who arrest after cardiac surgery. If you also provide your email address, we will provide you with a draft of the guidelines once the survey answers have been collated.

JCTSE Surgical Director of Education – Call for Applications
The newly organized Joint Council on Thoracic Surgery Education, Inc. (JCTSE) is seeking an ABTS-certified thoracic surgeon to serve in a half- to full-time position as its first Surgical Director of Education. The position has been created to direct the Thoracic Surgery Education Initiative formulated by the American Association for Thoracic Surgery (AATS), American Board of Thoracic Surgery (ABTS), Society of Thoracic Surgeons (STS), and Thoracic Surgery Foundation for Research and Education (TSFRE). The position's major focus will be to improve cardiothoracic surgical resident education and facilitate the redesign of the thoracic surgery resident training paradigm, as well as to develop innovative techniques for post-resident education.

Perioperative Blood Management Technologist Examination
The International Board of Blood Management (IBBM) will administer a certification exam for specialization in blood management techniques on August 13, 2008, immediately preceding AmSECT's 16th Annual Symposium on New Advances in Blood Management, Snow King Resort, Jackson Hole Wyoming. The examination is a prerequisite to certification by the IBBM as a Perioperative Blood Management Technologist. This exam is available to individuals who are currently performing techniques of clinical Autotransfusion and cell processing in the perioperative environment. In the July 2008 issue of The Annals, the Geneva group report their experience with a new biodegradable mitral ring, which has some obvious theoretical advantages. They note that mitral valve repair for rheumatic mitral valve disease in children is the preferred surgical modality, and that a recently introduced biodegradable ring has shown promising results and allows for growth of the native annulus. Two hundred and twenty children underwent mitral valve repair for rheumatic valve disease, predominantly insufficiency (90%). Fifty-seven patients (26%) had associated aortic valve insufficiency and 51 (23%) had tricuspid valve insufficiency addressed during the same surgery. Mitral valve rings were used in 213 patients (173 Carpentier-Edwards and 40 biodegradable rings). There were no hospital deaths or major postoperative morbidities, and follow-up (mean 74 months) was complete in 96% (212 of 220). Recurrent mitral insufficiency/stenosis-free survival was 94.5% at 5 years and 92.7% at 10 years. Mean gradient was 5.2 +/- 1.9, 6.2 +/- 2.0, and 7.0 +/- 2.3 mm Hg, respectively, at 7 days, 6 months, and 1 year postoperatively for the Carpentier-Edwards ring and significantly lower (p < 0.001) for the biodegradable ring at 2.8 +/- 0.5, 3.1 +/- 0.7, and 3.3 +/- 0.5 mm Hg, respectively. Unchanged mean gradient during the first year was 65% (26 of 40) for the biodegradable ring and 21% (31 of 147) for the Carpentier-Edwards ring. The authors concluded that the use of biodegradable mitral valve ring results in a significantly lower mean gradient during the first year of implantation compared with the Carpentier-Edwards ring.
The use of biodegradable rings intuitively seems like an excellent solution, and the data herein support the concept. The results overall, with either strategy, are excellent. Rings are not always necessary (according to some authors), which complicates the interpretation somewhat. This is an important contribution, and should stimulate some discussion and controversy.
