Management of Perioperative Anticoagulation
The new survey for the Thoracic Portal addresses an important topic in the management of non-small cell lung cancer. Patients with NSCLC also frequently suffer from cardiovascular disease and sometimes are on anti-platelet agents at the time of presentation for surgery. This survey will help inform the practicing surgeon about how other surgeons manage this challenging clinical situation.
These surveys are conceived and constructed by the Thoracic Portal Editor Dr. Mark K. Ferguson of Chicago. He will collate and analyze the results, and his comments will be posted in a future announcement.
To participate, go to the Thoracic Portal page and click on the Survey box in the right gutter. TAKE THE SURVEY NOW. IT TAKES LESS THAN 5 MINUTES!

East Carolina University dedicates Heart Institute
The dedication ceremony, held at the East Carolina Heart Institute at ECU, capped more than four years of work to bring a world-class cardiovascular disease institute to eastern North Carolina. In 2004, the N.C. General Assembly approved $60 million for a research, education and outpatient care facility at ECU. Pitt County Memorial Hospital (PCMH) secured private funding for a $160 million bed tower.
"What we dedicate today is a concept called the East Carolina Heart Institute that encompasses these new facilities, dedicated to the people of this region, and to the physicians, researchers, educators and staff devoted to their care," said Dr. W. Randolph Chitwood Jr., who is Chief of Cardiothoracic Surgery and Vascular Surgery in the Department of Cardiovascular Sciences at the Brody School of Medicine at ECU. He is also Senior Associate Vice Chancellor for Health Sciences at ECU.
The new buildings aren't the only sign of a new approach to heart disease for ECU and PCMH. The two organizations have also redefined their model for treating cardiovascular illnesses. Both have organized their clinical staffs around illnesses and disease processes, rather than following a traditional model based on academic specialties. That new model encourages more information sharing among doctors and puts new emphasis on patients' best interests, Chitwood said.
"This new model of care delivery is an innovative way to blend the science and art of medicine so that we add value to the patient," Chitwood said.
The prevalence of cardiovascular disease in North Carolina justifies the massive investment in treating and researching heart and vascular illnesses. Cardiovascular disease is the second-leading cause of death in the state, and nearly a quarter of North Carolinians suffer from cardiovascular ailments.
Planning for the Heart Institute dates to early 2003. ECU and PCMH broke ground for the institute in March 2006. In addition to the six-story 375,000-square-foot bed tower, the East Carolina Heart Institute at PCMH spawned a pair of companion projects: construction of a massive central utility plant to power the new building and the relocation of Moye Boulevard.
The East Carolina Heart Institute at PCMH has 120 cardiovascular beds, six operating rooms, seven interventional laboratories, three electrophysiology labs and a heart-healthy cafeteria, all designed to create the optimal patient care environment.
The four-story, 206,000-square-foot East Carolina Heart Institute at ECU houses scientific and clinical research, robotic-surgery training, space for simulation laboratories, a clinical outpatient facility for cardiovascular diseases, a database center, offices and an auditorium.

Shaping the Future of Mitral Valve Repair
The Profile 3Dâ„¢ ring is a fully rigid remodeling ring with a physiologic mitral valve shape and can be used to treat functional mitral valve diseases, such as ischemic mitral regurgitation and dilated cardiomyopathy.
CG Future® COMPOSITE Annuloplasty Ring: A unique composite design for dynamic mitral repair.
The orifice of a normal mitral valve is kidney-shaped and its transverse diameter is 33 percent greater than its anteroposterior diameter. In some diseased mitral valves, the annulus deforms so the anteroposterior diameter becomes equal to or greater than the transverse diameter altering the natural 3:4 ratio and resulting in mitral valve insufficiency.1
The CG Future® COMPOSITE Ring offers semi-rigid posterior remodeling of the mitral valve. A flexible anterior segment connects to the ring's semi-rigid posterior, providing the support needed to dynamically remodel the annulus, thereby allowing physiologic movement throughout the cardiac cycle.

New Book Offerings in CTSNet Bookstore
The following three books written by CTSNet cardiothoracic surgeons have recently become available in the CTSNet Bookstore:
(1) ICU Recall (3rd ed.) by Curtis G. Tribble, Charles E. Hodson, and Nelson L. Thaemert.
The updated Third Edition of ICU Recall facilitates rapid review and memorization with a concise question and answer format. Topics include ethics, pharmacology, radiology, and, especially, ICU-focused questions.
(2) Pioneers of Cardiac Surgery by William S. Stoney.
Heart operations today are quite common and relatively low-risk, but in the beginning it was just the opposite. Cardiac operations were reserved for desperately ill patients. The author documents this dramatic transition with profiles of 38 surgeons, active between 1940 and 1985.
(3) Video Atlas of Minimally Invasive Thoracic Surgery by Anthony PC Yim, Michael KY Hsin and Innes YP Wan.
The goal of this video atlas is to demonstrate a broad range of thoracic procedures which are typically performed using minimally invasive techniques. The text is supplemented with 2 DVD-ROMS.
--------
These offerings can be found on the Featured Title list, along with a number of other volumes of interest to all branches of cardiothoracic surgery.
The bookstore is a joint venture between CTSNet and Amazon.com, and offers the CTSNet readership optimal pricing on almost all volumes. We encourage you to use the CTSNet Bookstore for all your medical book needs.

Dr. Nasser K. Altorki, The New York Hospital and Cornell Medical Center, New York, NY USA, writes: CALGB has recently activated a randomized trial comparing lobectomy with limited resection for small (2 cm or less) peripheral NSCLC that is node-negative. It calls for randomizing nearly 900 patients. It was endorsed by all the co-operative groups, including AGOSOG and NCI-Canada. Unfortunately, accrual has been slow, and we would like to raise awareness of the trial since it is likely, when completed, to be an important contribution."
To those interested in participating, he writes: "A large randomized trial of lobectomy versus sublobar resection for early stage NSCLC has been activated throughout the US and Canada. Read the Trial Proposal, and see how you can participate."
REIMBURSEMENT IS NOW INCREASED TO $3000 PER ELIGIBLE PATIENT.

Do you practice routine prophylaxis against nasal S. aureus to diminish post-op infections?†Studies have shown that nasal decolonization of S. aureus in patients undergoing surgery can diminish post operative surgical site infections. The purpose of this survey is to determine our attitude towards this issue, specifically whether we as heart surgeons routinely provide prophylaxis against this potential source of infection or think that we should be doing so. To get further information on this problem, Cardiac Portal Editor Ed Savage has developed a short survey to plumb the thoughts of CTSNet cardiac surgical readers. PLEASE RESPOND. IT WILL TAKE ONLY A FEW MOMENTS.
