ABTS - Recent Announcements and Activities

New Congenital Heart Surgery Certificate
The Board will be offering a subspecialty certificate in Congenital Cardiac Surgery starting in 2009. For more information on the requirements and application process, Click Here.

SESATS
The publication of SESATS X has been delayed. The expected release date is now Fall 2009.

Due to the delay, the Board will be using SESATS IX for the cognitive components of the 2009 Maintenance of Certification process. In early August, the Board sent an email to Diplomates who have been approved for MOC in 2009 with instructions on how to obtain a free copy of SESATS IX. If you are a Diplomate who has been approved for MOC in 2009, but has not heard yet from the Board, please contact the Board office.

Due to the publication delay of SESATS X, Penn State College of Medicine has extended CME credit for SESATS IX through June 30, 2010.

Penn State College of Medicine designates this educational activity (SESATS IX) for a maximum of 70 AMA Category 1 Credit(s) TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

2010 MOC Exam
The first Maintenance of Certification secured exam will be given from September 13-25, 2010. The exam will be given in an electronic format at Pearson Vue Testing Centers, which are located nationwide. Diplomates whose certificates expire in 2012 are eligible to take the exam, which should take approximately 5-6 hours. Diplomates will be able to choose the day of their exam and their testing center. More information about the process will be forthcoming.

Certification Categories
As recommended by the American Board of Medical Specialties, the ABTS has recently redefined its clinically active and clinically inactive status. For a full description of these two categories, click on ABTS home page.

FDA Panel Recommends Approval of the Medtronic Melody® Transcatheter Pulmonary Valve under HDE

This month, the U.S. Food and Drug Administration (FDA) Circulatory System Devices Panel unanimously recommended conditional approval of a Humanitarian Device Exemption (HDE) for the Melody Transcatheter Pulmonary Valve. The panel’s recommended conditions of approval include a post-approval study, revision of the patient brochure and instructions for use, and implementation of a physician training and education program. The Melody Transcatheter Pulmonary Valve provides a less invasive means to address the effects of a dysfunctional pulmonary valve for children and adults with congenital heart disease.

Melody is on track to become the first transcatheter heart valve of any kind to receive FDA approval for use in the US; however, following the panel recommendation, the Melody Transcatheter Pulmonary Valve remains an investigational device in the US, where its use is limited to a clinical trial approved by the FDA. Approval of the Melody system as an HDE is under review by the FDA.

Attend the 2009 Legislative Advocacy Workshop

Register Now to Attend the 2009 Legislative Advocacy Workshop!

The STS Legislative Advocacy Workshop is back—just as Congress is in the midst of debating health care reform legislation. Designed specifically to develop leaders who can take a stand on health care policy, the timing for the Advocacy Workshop is perfect. Plan now to attend and help shape the health care debate on behalf of cardiothoracic surgery!

The 2009 Legislative Advocacy Workshop will be held September 13-15 in Washington, DC. The anticipated congressional calendar calls for the culmination of health care reform legislation to be the major order of business when elected leaders return from their August recess. The Workshop will provide an extraordinary opportunity for STS members to participate in the history-making debate in Washington over health care reform legislation.

The Workshop will kick off with a Sunday evening reception, followed by a day of discussions with congressional leaders and Obama administration policy makers. Following an in-depth update on critical issues, an advocacy training session will help participants convey the specialty’s message. Appointments will then be scheduled for Tuesday morning as attendees hit Capitol Hill to meet with congressmen and senators to discuss the challenges facing cardiothoracic surgeons. Click here for more particulars. For questions and additional information, contact Bob Kohlmeyer, Government Relations Coordinator, at rkohlmeyer@sts.org or (202) 481-1026.

Advances in Quality & Outcomes 2009 – Register Now

Surgeons and data managers interested in improving the quality of both data collection and patient care should attend the upcoming Advances in Quality & Outcomes: A Data Managers Meeting to be held September 24-26 at the Marriott Hotel and Marina in San Diego, California.

The AQO Conference, now in its sixth year, has become the definitive meeting for STS Database professionals. Participation at AQO, like participation in the STS National Database, demonstrates a commitment to quality improvement and is an investment in the future.

In 2009, AQO attendees will benefit from in-depth presentations by data managers and surgeons who will provide practical applications for data collection and achieving quality outcomes. With the national spotlight increasingly on the potential use of databases, AQO offers an important opportunity for data managers to bring home new perspectives that can benefit their employers.

Separate sessions will focus on each of the three Databases that comprise the STS National Database: Adult Cardiac, General Thoracic, and Congenital Heart Surgery.

The Conference offers excellent opportunities to explore how others are implementing new initiatives and achieving cost savings. AQO is also a chance to connect with exhibiting software vendors and explore new or improved applications.

The AQO Conference is designed for data managers of all experience levels, and again this year will include a special session for beginners. Primary data contacts and new data managers in particular, are encouraged to attend. As in years past, continuing education units will be provided for nurses, perfusionists, and health care quality professionals.

Registration information for the AQO Conference can be found here. For questions regarding AQO Conference registration, contact Mary Kate Murray, STS Meetings Coordinator, at (312) 202-5816 or mmurray@sts.org.

Zachariah JPV, Pigula FA, Mayer JE, Jr, McElhinney DB.
Right Ventricle to Pulmonary Artery Conduit Augmentation Compared With Replacement in Young Children.
Ann Thorac Surg 2009;88:574-580.

In the August 2009 Annals, Zachariah, et al, from Boston have presented data regarding conduits in the pulmonary circulation.  Although there have been countless detailed analyses of this subject over the past 20 years, the current group maintain that the interpretation of prior reports is difficult due to heterogeneity of the population and other reasons. They purport to clarify the subject and add data regarding conduit augmentation rather than replacement, an approach similar in principle to that proposed by Cerfoglio, et al, (Mayo Clinic) some years ago. The current report is quite interesting, although also difficult to interpret since the data are all retrospective and the indications arbitrary. Nonetheless, it is a very useful report.

Patients included children younger than 10 years of age undergoing a first conduit reoperation (n = 180), involving either conduit replacement (n = 147) or conduit augmentation (n = 33). There were no differences between the two groups with respect to age, size, or hemodynamic variables. Overall, early mortality was 0.5%, and 10-year survival was 95%. Freedom from reoperation was 80% at 5 years and 39% at 10 years, whereas freedom from reintervention (reoperation or catheter intervention) was 55% at 5 years and 26% at 10 years, with no differences between groups. As in other reports, undersized conduits were associated with shorter freedom from reoperation and both smaller BSA and undersized conduits were associated with shorter freedom from reintervention.

The authors concluded that conduit augmentation offers similar clinical outcomes in selected patients. There are some obvious implications for morbidity and cost reduction. Have the authors demonstrated that pulmonary insufficiency will not be a clinical problem as it is in other settings?  Add your own comments in the Dilemmas in Pediatric Cardiac Surgery discussion forum

The monthly articles for discussion are selected and summarized by Dr. Tom R. Karl, Mater Children's Hospital, Brisbane, Australia. Dr. Karl is Editor of the ATS Discussion Section.
