Submitral Aneurysm Repair

Submitral Aneurysm is a rare cardiac pathology with varied clinical manifestations. Etiology can be congenital defect in fibrous annulus of posterior mitral leaflet and/or inflammation.

This video presents a successful repair of a submitral aneurysm in a 29-year-old male patient, who presented with dyspnea on exertion and intermittent chest pain for the prior 10 days. 2D ECHO and TEE showed evidence of submitral aneurysm with severe mitral regurgitation. Coronary angiography was normal. The aneurysm was at the level of posteromedial commissure, extending down into the posterobasal myocardium, and up through the posterior mitral annulus into the left atrium. The patient presented with acute symptoms due to the rupture of the left atrial extension of the aneurysm. This video shows the preoperative evaluation, intraoperative findings, successful repair technique, and postoperative findings. On further follow-up, patient is asymptomatic and has no residual mitral regurgitation.
