This video shows the successful removal of a 10 cm anterior mediastinal mass. Initially, a sternotomy was considered due to the significant size of the mass. However, a camera was inserted into the chest to assess the feasibility of a video-assisted thoracoscopic surgery (VATS) approach. The resection proved technically straightforward using only a Hook LigaSure and a 5 mm grasper.
The primary challenge involved the extraction of the specimen. A subxiphoid incision was utilized, and the exit path was cleared from inside by clearing the thymus and surrounding tissue from beneath the sternum to ensure safe removal.
This approach utilizes a subxiphoid incision previously employed for “microlobectomy.” A technique that utilizes only 5 mm ports for the lobectomy, with the subxiphoid incision serving as the site for stapling, drain placement, and the removal of the lobe.
