Perfecting TAVR Removal | Skills Sharpening With Vince Gaudiani

The surgical operation to remove transcatheter aortic valve replacements (TAVR) and reconstruct the left ventricular outflow tract will become an increasingly important operation to perform well as more TAVRs are implanted. The initial results from performing this operation have been disappointing, with mortality rates exceeding 16 percent. More recent data showed a series with a 5.5 percent mortality rate, which is still considered high.  

Several factors contribute to these unsatisfactory surgical outcomes. First, it is a new operation, and surgeons have limited experience with it. Second, some patients, particularly those with ruptured annuli, come to the operating room in extremis. Third, this operation presents two separate challenges that are linked—safely removing the TAVR and reconstructing the left ventricular outflow tract, given the effects the TAVR has had on it.
