Demonstrated in the video is a novel concept for facilitating extensive neochordal repair in mitral regurgitation based on the paradigm “respect rather than resect”. Both leaflet tissue and motion are respected. The leaflet is folded and the neochordae is tied, maintaining free margin and annulus edge-to-edge.

There is increased interest in pursuing a robotic approach as early experience indicates some potential advantages, including less pain in the early postoperative period and possibly more complete hilar and mediastinal lymph node dissection.

Hemodynamically stable patients with an undiagnosed pericardial effusion are well suited for a thoracoscopic window. When pericardial, pleural, or pulmonary pathology co-exist and require diagnosis or therapy, a thoracoscopic approach is ideal.
