The PROACT trial is evaluating the clinical utility and safety of less aggressive anticoagulation schemes after bileaflet AVR. This arm of the trial compared low dose (INR 1.5-2.0) to standard dose (INR 2.0-3.0)warfarin.

The utility of dental extraction for infection prior to planned cardiac surgery is unkown. The authors evaluated the risk of dental extraction in a group of 205 patients for whom elective cardiac surgery was planned. 8% experienced major morbidity after dental extraction, 3% died prior to cardiac surgery, and 3% died following cardiac surgery.

Patients requiring readmission after cardiac surgery were compared to time-matched patients who did not require readmission to evaluate factors associated with this adverse event. Predictors of readmission included COPD, decreased EF, prolonged length of stay, and lower educational level.

Chest drainage following cardiac surgery is used to avoid complications related to the accumulation of blood and serous fluid in the chest. We aimed to determine the incidence of chest tube clogging and the role of bedside assessment in identifying the potential for failure to drain. CONCLUSIONS:

The authors report on patients with a small aortic annulus who underwent aortic root replacement using self-assembled valve composite grafts with prosthesis larger than aortic annulus. This technique resulted in excellent haemodynamic and good neo-root durability at long-term follow-up.

In this single centre retrospective review, the authors in a multivariable analysis determined the surgeon was a predictor of blood loss after cardiac surgery. Despite much research, bleeding and blood use remains an issue after cardiac surgery. Each surgeon should examine their own techniques to minimise bleeding risk.

Significant carotid artery disease is not uncommon in patients undergoing open heart surgery (OHS). Whether to treat both conditions in a staged or combined procedure remains an unresolved matter.

The authors present a case of a 63-year-old female who developed a large inferior aneurysm following late presentation of myocardial infarction. This rare presentation is discussed with specific regard to imaging and surgical intervention. This patient demonstrates that not all pseudoaneurysms rupture rapidly.

Left internal thoracic artery (ITA) grafting to the left anterior descending artery has long become the gold standard. There is a trend for bilateral ITA grafting, commonly to left-sided arteries. The video shows ITA skeletonization, with low harvesting time, and minimal chest and graft thermal trauma.
