This video shows an 85-year-old woman who presented to the emergency room with chest pain and a 6.5 cm ascending aorta containing an intramural hematoma and a pericardial effusion.  

The innominate artery was cannulated to perfuse and cool the brain before any further intervention.  The left carotid artery was clamped, allowing the brain to be cooled while keeping the body warm.  

The aorta was cannulated, opened, and a large tear was seen in the lesser curve of the aorta. The ascending aorta was replaced with a 30 mm Dacron tube graft after the aneurysm was resected.

Sir, what was your sequence of cannulation and your CPB setup. Could you kindly elucidate your protocol of cooling.....innonimate A to the brain perfused at 20 degs, and lower body at 30 degs...and as far as can be made out you clamped both the carotids , but what about the backflow from LSCA ?? Was the flow in the arch cannula shut off while doing the proximal and distal aortic anastomosis for 57 mins??
