This video features a patient who needed a left internal mammary artery (LIMA) graft to the left anterior descending artery (LAD) and a graft to the posterior descending artery (PDA). Due to the patient’s weight and height, 110kg and 5 feet 5 inches, selecting an appropriately sized valve was crucial.   

The aortic incision was continued between the left and right coronary cusps, descending onto the A2 cusp of the mitral valve.   

A flame-shaped Dacron graft was made, and the method for placing this as a patch is shown in the video. The first three sutures were placed, then it was parachuted down, and forehand continuous sutures were used on both sides. The placement of the valve sutures and placement of the valve is also shown in the video.
