The authors present the case of a 40-year-old male with no prior medical history who reported progressive dysphagia to solids over several months. Imaging with CT and MRI revealed a submucosal esophageal lesion consistent with leiomyoma. The patient underwent a right-sided robot-assisted thoracoscopic surgery (RATS) for tumor resection. Intraoperative methylene blue test showed no mucosal breach. The muscular layer was closed with interrupted 3-0 polydioxanone (PDS) sutures, and the adventitia and pleura were closed with a running 3-0 PDS suture. The postoperative course was uneventful. A contrast esophagram performed on postoperative day two showed no leak, and the patient was discharged on day three in good condition.
