Mitral Repair of a Calcified Posterior Leaflet With LIMA to LAD and Closure of PFO  [1]

Dr. Vince Gaudiani demonstrates how to perform a complex repair of a mitral valve with a heavily calcified posterior leaflet. 

He began by cannulating the femoral vein and then performed a sternotomy. Following this, he placed a right superior pulmonary vein vent and performed aortic cannulation with antegrade cardioplegia.   

The roof of the left atrium was then opened, and stay sutures were placed to optimize the view. Before assessing the valve, all the annuloplasty sutures were placed.  

Upon assessment of the valve, the posterior P2 was heavily calcified. Thus, the base of this leaflet was cut, and the leaflet was decalcified.  The leaflet was then positioned more appropriately in the ventricle and was sutured back onto the annulus.  Prior to placing an annuloplasty ring, the clefts were closed, and the chords were placed.
