Reoperation for Undersized Mechanical Mitral Valve With Paravalvular Leak and Residual MAC [1]

The patient had a 23 mm mechanical MVR and LIMA to LAD and SV to OM1, eight months prior, but developed a large paravalvular leak that caused hemolysis, requiring regular transfusions.
She was transferred to Dr. Vince Gaudianis’ hospital and underwent redo surgery. Femoral venous cannulation and direct aortic cannulation were performed, and the roof of the left atrium was opened.
The valve was too small for the patient, and she was suffering from prosthetic mitral valve stenosis in addition to a paravalvular leak.
This video demonstrates the removal of the previous mechanical valve, more extensive debridement of the posterior mitral calcification that had previously prevented the implantation of a larger valve, and the insertion of a 27 mm Epic biological valve.
