Severe leaflet tethering which is mainly the result of long standing Tricuspid valve regurgitation and RV dilatation is the main predictor of residual and/or recurrent valve insufficiency. We present the Novel technique of Papillary Muscle Cross Control which effectively overcomes the leaflet tethering in a very practical, simple , and reproducible manner by controlled approximation of the anterior papillary muscle to the posterior/septal part of the semi-rigid annuloplasty ring. The papillary cross-control technique is applicable both as a supplementary technique after ring annuloplasty for functional TR with tethered cusps and also for completing the repair in more complex cases of organic valve disease such as Ebstein Anomaly. Our early and late results with this technique has been quite encouraging.

A brief focused commentary on a recent medium size Japanese series of succesful surgery for Acute Aortic Syndrome. The limitations of the series presented in well- written Uchida manuscript( and already to the last AATS Meeting) are being explored, and the commentators' allusion to the need for randomised trial is evident.

A large randomised double-blinded trial reporting a considerable benefit ( 0.79 HR with very small p and supporting confidence intervals) of the Boehringer SGLPT-2 inhibitor regardless of presence of formal diagnosis of diabetes.

Scholarly endeavour to digest, in a relatively short and dense manuscript, the State-of-the-Art in assessing the function of the Forgotten Chamber. The notion of surgjcal pericardiotomy as a confounder of decision making is most interesting

This video shows the Ross procedure with a reinforced pulmonary autograft in an adult with aortic valve stenosis, secondary to a congenitally bicuspid aortic valve and dilated ascending aorta.

A scholarly narrative on the utility of quantifying the performance of the right ventricle: the RV has been hitherto empirically assessed in many clinical setups.

The technique of congenital aortic valve fenestration repair with valve-sparing aortic root replacement (root remodeling with extra-aortic ring annuloplasty) is presented in the case of a 24-year-old female with an expanding aortic root aneurysm, heterotaxy syndrome, and interrupted inferior vena cava.

A small meta-analysis suggesting a bemefit of vascular ultrasound for access to the groin vessels in TAVI. The utility of ultrasonography may be extended in access for groin cannulation in aortovascular surgery and ECMO

A comprehensive update on the evidence on the management of acute aortic syndromes, including IMH. Of particular interest is the robust recommendation to bypass non-dedicated aortic centers even to the expense of increased transit time in order to offer management in a Comprehensive Aortic Center (CAC).
