Bilateral Orthotopic Lung Transplantation After Prior Cut and Sew MAZE

Bilateral lung transplantation after prior cut and sew MAZE presents a challenging technical scenario because severe calcifications within the left atrium and pulmonary veins make it difficult to safely clamp and sew the pulmonary vein anastomosis. To safely perform the pulmonary vein anastomosis, the surgeons converted a typical venoarterial extracorporeal membrane oxygenation (VA-ECMO) approach to full cardiopulmonary bypass and arrested the heart. This allowed the authors to fully drain the heart and open the vein cuffs for debridement and completion of the pulmonary vein anastomoses.
