Determining the Individualized Probability of Myocardial Recovery: The Multicenter RecoverHeart Calculator

This multi-institutional study developed and validated the RecoverHeart calculator to estimate the probability of significant myocardial reverse remodeling during continuous-flow left ventricular assist device (LVAD) support. Using a derivation (n=509) and a validation cohort (n=375), the researchers defined reverse remodeling objectively (left ventricular ejection fraction ≥40 percent and end-diastolic diameter ≤6 cm within one year of LVAD implantation) rather than relying on subjective explantation rates. Adaptive regression identified female sex, non-ischemic etiology, shorter heart failure duration, and smaller preoperative left ventricular diameter as key independent predictors of recovery. The prognostic tool demonstrated robust discrimination (validation C-statistic 0.75), significantly outperforming prior models, thus utilizing a more objective recovery prediction model that could mitigate clinician biases, encourage LVAD bridge-to-recovery utilization, and theoretically reduce transplant waitlists. In summary, the RecoverHeart calculator reliably stratifies reverse remodeling potential to guide shared decision-making for advanced heart failure therapies.
