In this two-center Italian case series of six beating-heart transplants from controlled donation after circulatory death (DCD) donors, thoracoabdominal normothermic regional perfusion restored satisfactory biventricular function and enabled in situ graft assessment. Furthermore, beating-heart procurement and implantation under continuous normothermic coronary perfusion yielding 100 percent 30-day patient and graft survival, no severe primary graft dysfunction, and preserved biventricular function at discharge, supporting further evaluation of this ischemia-sparing strategy in larger studies.
