Torsten Doenst of Friedrich Schiller University, Jena, Germany, discusses surgical strategies for challenges faced during minimally invasive mitral valve surgery. Dr. Doenst describes a variety of techniques for dealing with cases of mitral annular calcification, significant peripheral artery disease, and severe aortic regurgitation/stenosis.

1. With respect to “mitral challenges”, did you considered high-risk patients?. I mean high risk assessed by EUROscore or STS score.
2. Did you perform preoperative thorax CT in patients with previously sternotomy?.
3. Do you considered that MICS is a good choice in patients with active endocariditis, reoperations and high risk patients?
