Posterior Uniportal VATS for S6 Resection and Completion Lobectomy

A 65-year-old female patient was admitted to the author’s clinic for the treatment of a predominantly lepidic adenocarcinoma located in the superior segment of the right lower lobe. Due to the ideal location and size, anatomical segment resection and radical lymphadenectomy was planned.

A fissure-last, video-assisted thoracoscopic (VATS) resection of the superior segment of the right lower lobe was performed using a 3 cm uniportal posterior approach, and was followed by a mediastinal lymphadenectomy. The frozen section analysis revealed microscopic presence of lepidic tumor on the resection margin, so a completion lower lobectomy was performed. The postoperative course was event free and the patient was discharged on the fourth postoperative day. The 1.8 cm diameter tumor was diagnosed as a primary lepidic (80%) and acinar (20%) adenocarcinoma staged pT2aN0M0.
