This video shows an experimental method for total reconstruction of the mitral valve with autologous pericardium. The author believes that this method may become an alternative for prosthetic valve replacement in cases where mitral valve repair is not feasible.

Congratulation for the beautiful images recorded. It would be interesting to see how you cut and shape of the pericardium. Do you think that in clinical practice this stentless valve should be associated with a flexible prosthetic ring?
Dr. Vanelli, thank you so much for your comments. As I tell in the video`s narration, the special molds should be used to make the intra-operative tailoring of the pericardium easy, fast and reproducible. I have invented such molds and have their design patent. They are different sizes molds (26, 28, 30, 32, 34, 36, 38 and 40 mm), same sizes as for the annuloplasty rings. All we will need is to choose the appropriate size mold according to the patient`s inter-commisural distance. Yes, I think that in clinical practice this valve should be associated with a prosthetic ring to make the fibrous annulus stable and prevent its dilatation.
Dr. Mazzitelli, thank you very much! I have seen your video dedicated to the total autologous reconstruction of the aortic valve using Dr. Ozaki`s method. Please be advised that I have developed my method of intra-operative tailoring of the pericardium for this operation using very simple formulas without need of any mold and templates. This method such operation much easier and more reproducible. Please see my video dedicated to the total autologous aortic valve reconstruction published in CTSNet. I would be pleased to know your opinion and collaborate with you in this area.
Radu Deac had described using autologous pericardium to fashion an anutologous pericardia;l patch. I have done quite a few cases using that method. One bug bear with all such methods is the integrity of the papillary neo valve connection that can in an occasional patient give waynecessitating emergency redo surgery but if it heals well they ahve an excellent short and midterm outcome.
I am very pleased to tell that Dr Feridoun Sabzi from Iran has recently operated 4 patients using my method of mitral valve autologous reconstruction. The early follow up results are very encouraging. Our article will be published in the American Journal of Cardiovascular and Thoracic Surgery 2020, Volume 5, Issue 1.
