SCTS 2025 | Patient-Prosthesis Mismatch in Aortic Valve Intervention

In March 2025, the Society for Cardiothoracic Surgery in Great Britain and Ireland held its annual meeting, bringing together professionals in the field of cardiothoracic surgery to discuss the latest advancements in technologies and techniques. At the Society for Cardiothoracic Surgery in Great Britain and Ireland Annual Meeting, leading surgeons presented on various aspects of patient-prosthesis mismatch (PPM).  

Dr. Rebecca Gosling, NIHR clinical lecturer at Sheffield Teaching Hospitals NHS Foundation Trust, addressed the “Clinical Implications of Patient Prosthesis Mismatch—UK Experience.” She explored the diagnoses of PPM, predictors of postoperative gradients, and offered insights into future research in this field. 

Dr. Madalina Garbi, consultant cardiologist at Royal Papworth Hospital, Cambridge University Health Partners, presented on “Aortic Valve—Measurement and Diagnosis.” She discussed PPM prediction algorithms, various PPM apps, and the standardization of valve charts.  

 Dr. Norman Briffa, a cardiac surgeon at Sheffield Teaching Hospitals NHS Foundation Trust, spoke on “Surgical Approaches and Solution to Mitigate Mismatch.” He discussed different types of prostheses and root enlargement procedures, such as Manouguian and Nicks techniques.   

Dr. Syed Sadeque, consultant cardiologist at Sheffield Teaching Hospitals NHS Foundation Trust, delivered a presentation titled “Y Annular Enlargement: Tips and Tricks.” He shared insights on bypass settings, cardioplegia issues, patch selection, measurements and valve considerations, and strategies to minimize bleeding risk.  

Dr. Dominik Schlosshan, consultant cardiologist and clinical lead for cardiology at the Yorkshire Heart Centre, Leeds General Infirmary, presented on “Postoperative Management in Mismatch.” He emphasized the importance of baseline echocardiograms, the importance of follow-up on PPM, its effects on the heart and the patient, and the timing of interventions.
