Aortic Valve Replacement 101 Through an Upper Ministernotomy

This video describes Dr. Vince Gaudiani’s technique for a standard aortic valve replacement. The patient was a 69-year-old man with severe bicuspid aortic valve stenosis.

To begin, femoral vein cannulation and direct aortic cannulation were performed. The aorta was then opened, the valve was removed, and a 27mm Edwards bioprosthesis was used to replace the valve.

The use of pledgets was discussed, with some pledgets coming from the outside of the aorta.  The valve was parachuted into place, and Cor-Knots were used to secure the valve. The aorta was then closed with a single layer of 3-0 Prolene. The bypass time was 42 minutes, and the cross-clamp time was 35 minutes.
