This video demonstrates a robotic left lower lobectomy using bedside stapling to improve efficiency and assist with the rapid flow of the operative procedure. Interestingly, with the Si system, this was the routine way of performing the operation, but with the advent of robotic stapling, interest in bedside stapling has declined. However, the advantages of bedside stapling are numerous.
First, with more junior surgeons or trainees, if the senior surgeon is scrubbed, they can significantly speed the cadence of the operation by performing the stapling. Second, when bedside stapling is performed, no instruments need to be removed or exchanged, which improves the flow of the operation. Additionally, the best available staplers can be used, including reinforced staplers and 60 mm staplers, to improve the aerostasis and expedite the stapling process, especially for long fissures. Furthermore, when the stapler is advanced, the surgeon has two robotic hands available to maneuver the lung into the stapler rather than losing one arm to control the stapler.
