A giant mediastinal mass has traditionally been operated on using an open approach. However, a minimally invasive approach can be done with good results. Robotic surgery has been proven safe for mediastinal mass removal. In this case, the decision to operate by RATS was influenced by the surgeons’ expertise in this field, being a high-volume center, and having operated on similar cases before with excellent results.
A less invasive approach is associated with better recovery and less pain. In addition, RATS offers the benefits of articulated graspers, which allows surgeons to mimic open gestures in a close surgery. In association with capnothorax, it offers an excellent view and precise dissection. This has the potential to allow the surgeon to come back with a rescue surgery in case of recurrence with fewer complications, avoiding a re-sternotomy.
