In this video, Dr. Vince Gaudiani demonstrates a simple triangular resection of the mitral valve for a sixty-eight-year-old male with P2 prolapse. He uses an upper hemisternotomy with a femoral vein cannula and direct aortic cannulation, and a left atrial vent with antegrade cardiolplegia. He also shows how to access the mitral valve through the roof of the left atrium, which allows the upper hemisternotomy approach.
To begin, the resection of the valve is performed and then the annuloplasty ring sutures. The incision in the valve is then closed with an interrupted 4.0 Prolene. A pericardial patch is used to reinforce the base of the triangular resection and a 30mm Duran partial ring is placed to complete the operation.
