10-Year Outcomes of SAPIEN 3 Transcatheter Aortic Valve Replacement or Surgery in Intermediate-Risk Patients

This propensity-matched analysis compared 10-year outcomes after SAPIEN 3 transcatheter aortic valve replacement (TAVR) in the PARTNER 2 SAPIEN 3 Intermediate-risk Registry with surgery in the PARTNER 2A trial. Among 2,005 valve-treated patients, 1,069 underwent TAVR and 936 underwent surgery; after matching, 783 patients were included per group. The mean age was approximately 82 years, 43 percent were female, and the mean Society of Thoracic Surgeons (STS) score was 5.5 percent. At 10 years, all-cause mortality was similar after TAVR and surgery (83.4 percent vs 82.3 percent; hazard ratio [HR] 1.01, 95 percent confidence interval [CI] 0.91–1.13; p=0.82). Aortic valve reintervention was also similar (2.0 percent vs 1.9 percent; p=0.47). The authors conclude that SAPIEN 3 TAVR and surgery had similar 10-year mortality, reintervention, and hemodynamic outcomes.
