This paper argues that trust, transparency, fairness, psychological safety, and workforce stability are not soft idealsthey are operational duties of surgical leadership. In an era of consolidation, productivity pressure, administrative harm, and workforce strain, the specialty must protect patients while also treating surgeons and their teams with dignity. Surgical excellence cannot flourish in an environment where mentorship, fairness, and trust are allowed to erode.
