In this retrospective study of 170 patients undergoing pulmonary thromboendarterectomy for chronic thromboembolic pulmonary hypertension at a high-volume center, 25 percent developed recurrent moderate or greater tricuspid regurgitation. Reoccurrence was associated with preoperative and residual tricuspid regurgitation (TR) severity, higher predischarge pulmonary vascular resistance, and impaired right atrial reservoir strain; however, not tricuspid annular diameter. These findings identify right atrial reservoir function as a novel predictor that may inform decisions about concomitant tricuspid repair and postoperative surveillance.
