Across 252,408 angiograms performed by 40 interventional cardiologists in British Columbia (2010–2024), the proportion of patients referred for coronary artery bypass grafting (CABG) varied 13-fold (2.03 percent–26.4 percent) and was not explained by hospital factors alone. Higher referral rates correlated with lower percutaneous coronary intervention use, less extensive PCI, and lower procedural volume. Among 73,603 first-time revascularized patients, higher CABG rates predicted fewer repeat revascularizations without affecting mortality or major adverse cardiovascular events. Wider Heart Team use may improve consistency.
