A sixty-five-year-old woman developed double vision and the investigation she underwent revealed an 11 cm thymoma. Subsequently, she had a median sternotomy and thymectomy, but the left phrenic nerve was injured, which resulted in left diaphragmatic paresis. The surgery was followed by radiation of the anatomic area of the thymus, as histology showed a type B3 thymoma.
Recently, the symptoms of the myasthenia reappeared and a new investigation showed opacities on the left chest. A PET scan confirmed the avidity of these lesions. After discussion, it was decided to proceed with excision of the metastases. Although the patient previously underwent open surgery and radiotherapy, she was offered a robotic-assisted approach with an opportunity to fix the diaphragm at the same time through plication. The procedure was successful, and the patient was discharged 24 hours post-surgery.
