Hemofiltration During Cardio-Pulmonary Bypass in Coronary Artery Surgery
Wythenshawe Hospital and Manchester Royal infirmary (Prof Ian Hutchinson and Mr N Yonan)
§ The study was designed to find out the potential benefits of Hemofilter on the outcome of coronary bypass patients in adults, and what is the mechanism of such potential effects.
§ It was a prospective randomized trial in which 79 first time CABG patients were recruited. They were randomized into 2 groups’, group A with Hemofilter attached to the arterial side of the heart lung machine and group B as a control group.
§ Leader in this project, I was responsible for recruiting and consenting patients, collecting all the relevant data, taking blood samples and processing them, conducting the lab work, entering the data on a computer spread sheet and carrying out the statistical analysis of the results. I collected all the relevant clinical preoperative, intraoperative and postoperative data.
§ It was proved that Hemofilter use during cardiopulmonary bypass improves the outcome of heart surgery in children. This improvement was found to be mediated through affecting the levels of the cytokines.
§ Papers generated out of the research:
Ø Renal Injury after Cardiopulmonary Bypass is Associated with Elevated Levels of Cytokines IL6 And IL10 and Hemofilter Use Has No Effect on Post Operative Kidney Function.
This paper was presented at the 21st European meeting in Geneva in September 2007 and published by the European Journal of Cardiothoracic surgery in March 2009.
Ø Use of Hemofiltration During Cardiopulmonary Bypass has No Effects on the Incidence of Atrial Fibrillation After Coronary Bypass Grafting Surgery
Presented at the 18th Congress of the world society of Cardio-Thoracic Surgeons meeting in Greece, May 2008.

Additional Educational Information
§ I worked as assistant professor of cardiac surgery at the medical school of Jordan University for Science and Technology where teaching of medical students and surgical residents was an essential part of my duties as well as attending the post graduate teaching activities of junior doctors.
§ During my training I organized the postgraduate teaching activities in. Also I prepared and presented the following topics: Timing of CABG after Acute MI, Timing of Valve operations, Anticoagulation after Cardiac Surgery; How to minimize bleeding after Cardiac Surgery, Conduits for CABG, Prophylactic Antibiotics in Cardiac Surgery; HITT after CABG, and Management of Cardiothoracic Emergencies. The audience consisted of consultants, junior doctors, nursing staff and physiotherapists.
§ I have supervised three wet labs in our department [coronary anastomosis, mitral valve repair and aortic valve replacement].
§ I introduced the CALS course to our unit at Manchester Heart Centre and I was a member of the Faculty of this course.
§ Responsible for teaching of SHOs and the surgical assistants on Thursdays following the ward round in Manchester Royal Infirmary.
§ Retain a good collection of teaching materials as x- rays, CT scans, ECGs, PFT and angiograms that I use in my teaching. In fact, all my training was in teaching university hospitals, and teaching of medical students was always an essential part of my job.
§ In the theatre, I have supervised and trained registrars to do CABG cases. I also trained the juniors to perform sternotomy, thoracotomy incisions, dissection of LIMA, cannulation, constructing top ends and bottom ends, and closing of sternotomy and thoracotomy wounds, re-exploration for bleeding, how to insert a chest drain and how to harvest a vein.
§ I enjoy teaching during ward rounds. I explain symptoms, show signs, read x rays and other investigations, discuss the cases and plan the management in an interactive.
§ I attended a “Teaching the Teachers” course at Manchester University. This greatly
Improved my presentation skills. I also constantly seek to improve my technique by
Observing other presenters and by obtaining feedback from my audience.
