Kaluwelle Mudalige Vishva Alles MBBS MD Surgery MRCS UK Glasgow
Kaluwelle Mudalige Vishva Randhara Alles MBBS MD Surgery MRCS UK Glasgow, Senior Registrar In Cardiothoracic Surgery
Background
Personal Statement.
Currently I am working as a Senior Registrar in Cardiothoracic Surgery, which is equivalent to ST 7-9 Specialist Registrar in the UK, I am completing the last two months of the senior registrar period. I have altogether 10 years of experience as a doctor. Out of that, nearly 7 years are in cardiac surgery alone. During this period, I have had the opportunity to work with staff members and patients of different socio-cultural backgrounds and ethnicities. I have observed and understood how important this experience is to achieve better patient care and outcomes.
The National Hospital of Sri Lanka, Colombo, is my main training center in cardiac surgery. It is the largest and the main cardiac surgical unit of Sri Lanka. Sri Jayewardenepura General Hospital, Nugegoda, Colombo, Sri Lanka, is my other training center, which is also a leading high-volume cardiac surgical center in Sri Lanka. The cardiac surgical center of the Lady Ridgeway Children’s Hospital was my training center for pediatric cardiac surgical training, which is the largest center of its kind in Sri Lanka. I have gathered thoracic surgical training at the National Hospital for Respiratory Diseases at Welisara, Sri Lanka. It is the main and the largest thoracic surgical facility in the country.
I have had the dream of becoming a cardiothoracic surgeon. I planned it since the inception of my career as a doctor. I joined Sri Jayewardenepura General Hospital cardiac surgery unit as a junior doctor (equivalent to ST-2 of the UK) in 2014, gathered many experiences, and gradually became a senior doctor (equivalent to ST 3-6) in cardiac surgery within nearly 5 years. During this time, I had significant exposure and experience in pre-operative and post-operative care and surgical techniques in both on-pump and off-pump cardiac surgeries and also in minimally invasive cardiac surgeries. I have done graft harvesting, worked as the 1st assistant to the surgeon, managed emergency re-openings, and served as the 1st on-call person for cardiac surgical wards during this time. I carried out several research projects at that time.
In 2019, I decided to upgrade myself further by starting my postgraduate training in MD - Surgery. I have worked as a registrar in surgery (equivalent to ST 6-7) in many different surgical disciplines from 2019 to 2022. Since 2022, after obtaining my MD Surgery qualifications, I have been working as a Senior Registrar in Cardiothoracic Surgery. With the guidance of my beloved teachers and supervisors, I have now become an independent cardiac surgeon.
I have been involved in several research projects and audits, with some still ongoing. I tend to focus on research that involves lab work, which sometimes limits my ability to participate in a wider variety of studies. However, I was part of a research team that earned the President's Award of Sri Lanka for our work with lab works . Even the research proposal I submitted to the PGIM, University of Colombo, for my board certification involves performing platelet function tests on patients- lab works .
I enjoy teaching. I have started teaching since I worked as a demonstrator in Department Of Anatomy, University of Sri Jayewardenepura. I tech medical students, Junior doctors, pre MD post graduate registrars ,Nurses etc .I have worked to uplift the quality of the services we provide as cardiac care unit. For that I have coordinated several donations into our units. Conducting few audits to uplift the level of care.
On year 2023 I got elected as a council member in ACTSSL, I am working hard to uplift the cardiothoracic profession in Sri Lanka work in collaboration with other senior cardiothoracic surgeons of Sri Lanka.

Ongoing Audits /Studies and Researches
14. Audit on GSV Harvesting Wound inspection and bleeding After Applying Compression on Post Protamine: The data is currently being analyzed and will be submitted to the upcoming ACTSSL academic sessions 2024 January. Will complete the audit cycle by in the coming future .
More Notes on This Audit :
One method where my unit was practicing was primary closing the GSV harvesting site while the patient on Heparin effect and applying compression. This sometimes leads to Hematoma formation and wound infections. Instead of that we closed the wound primarily and applied only a temporary compression . Once the Heparin effect was reversed at the end of the surgery the compression was reapplied after squeezing the blood inside the wound and audited the degree of wound infection and hematoma formation and bleeding from the GSV harvesting site .
15. Study on Risk Factor Analysis of CABG Patients: This study is ongoing, and data collection is currently in progress.
16. Audit on Extubating Patients Following On-Pump CABG: This audit is also ongoing, The data is currently being analyzed and will be submitted to the upcoming ACTSSL academic sessions 2024 January.
More Notes on this Audit :
We have observed our CABG patients are taking much time to get extubated. Even more straight forward surgical patients are getting delayed to extubated .we have discussed this matter , have identified multiple clinical and non-clinical variables which are affecting this .The audit is still progressing
17. Study on Sri Lankan patients Awareness of Post-CABG Cardiac Rehabilitation: Data collection for this study is ongoing.
18. Study on Dental Hygiene and CABG/PCI Incidents in the Sri Lankan Population: This study is ongoing.
19. Study to Evaluate the Effectiveness of the Duration of Preoperative Aspirin and Clopidogrel Cessation in CABG Patients in Relation to Postoperative Bleeding and Blood Transfusion, Correlating It to Antiplatelet Resistance in the Sri Lankan Population:
This study is awaiting ethical approval and research grant funding. It needs lot of funds as we are planning to perform platelet function tests on patents . This proposal has been submitted to PGIM, University of Colombo, as part of the board certification study.
