The number of ports used in minimally invasive cardiac surgery can be reduced to one with the assistance of the latest technologies and a custom-made instrument. The authors demonstrate a novel single incision technique, where no other ports for instruments and no small incision for femoral cannulation is required.

Professor Robert J. McKenna, Jr., MD is Medical Director of Thoracic Surgery and Trauma at Cedars-Sinai, Los Angeles, CA. He pioneered the anterior approach for VATS lobectomy and has by far the largest experience in this technique. However, he has also served as the principal investigator on several trials, including the Z0030 trial on lymph node staging and also the NETT Trial.

Midesophageal diverticula are rare esophageal problems usually associated with granulomatous diseases and esophageal motility disorders. They are typically located at the subcarinal area with the wall of the diverticula adherent to the lymph nodes.

For purposes of this survey, readers were asked to assume that they consulted on an otherwise healthy middle aged patient with a distal esophageal adenocarcinoma, T1bN0 by EUS. There were no contraindications to esophagectomy.

A right robotic thymectomy is described for the treatment of myasthenia gravis and thymoma.  Early results are comparable to sternotomy and VATS approaches.  A novel technique using ICG for contralateral phrenic nerve identification is also described.

Induction chemoradiotherapy is a relative contraindication to VATS lobectomy due to oncological concerns. This video demonstrates a VATS lower lobectomy and radical systematic lymphadenectomy in a 53-year-old male who recurred locally following induction chemoradiotherapy.
