Lung - Metastases

May 4, 2023
A systematic review of prognostic factors for five-year survival after pulmonary metastasectomy (PM) from colorectal cancer (CRC) identified 11 of 77 preoperative factors as having statistically significant positive effects. These included a solitary metastasis < 2 cm.

The PulMiCC is a randomized clinical trial that tried to build more evidence for performing pulmonary metastasectomy in patients affected by advanced colorectal cancer and previously treated with curative intent. Unfortunately, the trial was stopped earlier becasue of poor recruitment and high drop out rate.

The author demonstrates a uniportal fissureless approach to resect the anterior segment of the right upper lobe, followed by lymph node dissection.

How to manage solitary pulmonary nodules appearing after treatment of a prior malignancy remains a clinical challenge.  The authors found that such patients most often had intrapulmonary metastases and less often had new lung cancers.  Factors associated with new lung cancers included spiculated nodules, absence of nodal involvement with prior cancer

Chudgar and colleagues evaluated clinicopathologic variables and characteristics of treatment for 539 patients who had undergone pulmonary metastasectomy for soft-tissue sarcoma at the Memorial Sloan Kettering Cancer Center.

Differentiating a primary lung cancer from an intrapulmonary metastasis can be challenging.  The authors developed a lineage test using whole genome amplification and next-generation sequencing to identify breakpoints in known primary tumors and known metastases.  Independent primary tumors did not share any genomic rearrangements, whereas primary tu

This phase II trial evaluated outcomes of isolated lung perfusion with Melphalan and complete surgical resection for pulmonary metastases from colorectal cancer or sarcoma in 50 pts.  Surgical mortality was 0%, but morbidity (primarily grade 3 or 4 pulmonary morbidity) was 44%.  30 patients suffered local recurrence.  3-year survival was 57% and recu

A prospective multicentre study analysed 532 patients with a first surgical procedure for pulmonary metastases of colorectal carcinoma. Complications arose in 16%, reoperations were necessary in 1% and mortality was 0.4%.
