Aortic valve disease (congenital)

October 13, 2017
The authors report on the design and use of a growth-accommodating device that may have a variety of pediatric applications, including valve annuloplasty. The device consists of a biodegradable core and a tubular braided sleeve (woven much like a 'finger trap' toy).

Balloon aortic valve dilatation has been assumed by some to provide the same outcomes as surgical aortic valvuloplasty. However, the development of precise modern surgical valvuloplasty techniques may result in better

The impact of combined mitral stenosis and aortic atresia on outcomes of Norwood procedures for hypoplastic left heart syndrome was examined in a single institution.  The incidence among 74 operated patients was 19%.  Mortality for affected patients was 29% vs 7% for the other patients.   The mechanism of mortality appeared to be myocardial ischemia.

In this surgical video, a modified Konno for recurrent discrete subaortic stenosis is shown. The indications for surgery were a gradient of 60 mmHg and onset of mild AR.

Aortic valve repair has encouraging mid- and long-term results in selected patients. This video describes the tailored cusp extension valvuloplasty surgical technique.

In this manuscript, the authors describe their findings after reviewing 27 young patients (between 7 and 30 years of age) who had undergone aortic valve replacement with two types of bovine pericardial biological prosthesis: Mitroflow® LXA (15 patients) and PERIMOUNT Magna® and Magna Ease® (12 patients) valves.

This video shows a teenager with a large neo-aortic root aneurysm and severe aortic valve regurgitation following a Ross procedure, which was treated with a valve-sparing aortic root replacement. The case also highlights the intra-operative management of inadvertent aortic injury during reentry in the setting of severe aortic regurgitation.

The authors demonstrate the steps for three-patch repair of congenital supravalvar aortic stenosis in a 5 month old.

The lack of an ideal valve substitute in the congenitally affected aortic valve has raised the interest in aortic valvuloplasty by leaflet extensions. Several techniques using native or xenopericardium have been described for aortic valve repair, however, the ideal material for leaflet extensions still remains controversial.
