Endovascular

August 14, 2014
This double-blind placebo-controlled trial involved 153 pts undergoing elective endovascular aortic aneurysm repair, of whom half received methylprednisolone preoperatively.  The intervention reduced the systemic inflammatory response from 92% to 27%, plasma IL-6 was decreased by 90%, and CRP, body temperature, IL-8, and TNF were also reduced.  MMP-9

Stepped wedged cluster randomized control trial from the World Health Organization comparing outcomes in those who received a Checklist.  Implementation of the WHO Surgical Safey Checklist decreased morbidty and hospital length of stay.

This report summarizes outcomes in 40 reoperated patients (mean age 33 years) of an original 122 who underwent initial aortic root surgery for Marfans. Reoperations took place a mean of nearly 10 years later. 5 and 10 year survivals after second operations were 80% and 66%. Open and stent grafts procedures did not appear to have different outcomes.

Computational models based on data from computed tomography show that maximal wall shear stress is associated with the maximal diameter in large aortic aneurysms whereas eccentricity of the bulge is more important in small aneurysms.

Aorto-oesophageal fistulation following thoracic endovascular aortic repair occurred in 36 of 2387 patients with a mean interval of 90 days according to the European Registry of Endovascular Aortic Repair Complications. Key symptoms included fever, haematemesis and shock.

Transaortic valve replacement is an alternative to the transfemoral and transapical approaches in an 83-year old male with ischemic heart disease, severe peripheral artery disease and severe symptomatic aortic stenosis.

Transapical aortic valve implantation (TAVI) has become a feasible alternative to open techniques in high risk patients. Surgical technique and some critical tips and pitfalls are described.

A technique for the treatment of a complex aortic disease is demonstrated in this video.  The aortic arch was replaced followed by deployment of a thoracic stent-graft through the branch of a Dacron graft.

Acute Type A aortic dissections can be challenging entities for cardiac surgeons and require close follow-up secondary to their propensity for aneurismal degeneration. A combined open and endovascular technique is described for treating the inital dissection and resultant complications.
